Lead poisoning is a non-infectious condition that healthcare professionals must report to the CDC as a public health practice. Preventing childhood lead exposure is the best course of action and this can be done by removing lead hazards from the environment.

Most children with lead in their blood have no obvious symptoms, so a blood test is the best way to determine exposure levels. Thankfully, blood lead levels in the U.S. population have declined over time, even in the most high-risk areas, in large part due to Federal policies and a coordinated effort of public health interventions. However, CDC lowered the blood lead reference value in October 2021 to identify children with higher levels of lead when compared to most children. About 4 million blood lead test results are received by CDC each year. Yet, CDC recommends that states develop statewide blood lead screening plans based on local data and conditions and (https://www.cdc.gov/nceh/lead/default.htm).

Patients and their safety are at the core of The Compliance Team Inc.’s (TCT) accreditation programs, which are built based on what matters most to patients, Safety-Honesty-Caring®. Durable Medical Equipment (DME) providers of home oxygen therapy (HOT) are required to perform home assessments and provide patients/caregivers with instruction and documentation on the proper placement, storage, maintenance, and transportation of oxygen, as well as the importance of not smoking.

TCT recently received a letter from the IAFC - International Association of Fire Chiefs regarding the increasing prevalence of fires due to HOT and the resulting incidence of (preventable) death and injuries among patients, family members, and firefighters. A report by the National Fire Protection Association (NFPA) noted one patient dying every four days (Hall, 2023) and two firefighters each year. An IAFC position statement in July 2022 cited a 14% increase per year in HOT-related burn injuries in a decade-long national database review. Smoking was the primary ignition source in 83% of these injuries.

Among the estimated 16 million people who have been diagnosed with Chronic Obstructive Lung Disease (COPD) in the US, an estimated 1.5 million are prescribed home oxygen. While difficult to measure, it is estimated that 10%-50% of smokers who receive HOT continue to smoke due to the addictive nature of ni****ne (American Thoracic Society, 2018). Oxygen-enriched fires create more deadly fire conditions and stopping the flow of oxygen reduces risks.

The IAFCs’ Fire & Life Safety Section (FLSS) is partnering with the U.S. Centers for Medicare and Medicaid Services (CMS), COPD Foundation, university hospitals and clinics, burn units, member fire-fighter associations, fire departments, advocacy groups, and home health medical equipment providers and distributors to reduce burn deaths and injuries. Their goals include reducing home oxygen fire deaths by 12% and saving nearly $500 million in burn care expenses while improving safety and quality of life.

FLSS and their partners support the use of newer thermal fuse technology for high-risk patients who are non-compliant smokers to reduce and potentially prevent HOT fires. An inexpensive FDA listed medical device manufactured in the U.K. and distributed in the U.S. by multiple independent medical supply wholesale companies is available. In 2018, the Veterans’ Health Association required a bidirectional thermal fuse/shut-off device be installed on all HOT systems involving veterans when it issued a Patient Safety Alert and advice to DMEs.
