The registered nurse plans, organizes, and directs home health services. The RN functions as a case manager, coordinating care between disciplines and MD's. Home Health experience required for this position.

Essential Job Functions/Responsibilities:

Completes an initial assessment of patient and family to determine home care needs.
Provides complete physical assessment and history.
Regular re-evaluates patient nursing needs.
Initiates plan of care.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic preventative, and rehabilitative nursing actions. Includes the patient and family in the planning process.
Initiates appropriate preventative and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate.
Identifies discharge planning needs.
Acts as a Case Manager as assigned by the clinical supervisor.
Prepares clinical notes and updates physician.
Communicates with the physician regarding the patient's needs.
Communicates with community health-related persons to coordinate care.
Participates in on-call duties as defined by the on-call policy, 1-2 weekends a month depending on needs of company.
Ensures that arrangements for equipment and other necessary items and services are available.
Instructs, supervises, and evaluates home health aide care.'

WHY DO YOU LOVE HOME HEALTH?
“I love being able to spend one-on-one time with patients, not being pulled in so many directions at once like in a hospital setting. My favorite patient was a sweet lady who had broken her hip. I got to walk her through her post-op care during the start of COVID, all the way back to her prior level of independence: laughing and smiling back home.”
-Paige received her AS in Nursing from Yakima Valley Community College, and her BS in Nursing from The University of Providence.

Our Services: Social Work
Our Social Workers focus on taking advantage of the appropriate and available community resources so that patients have the best chance of staying safely in their own homes. Social workers may identify needed medical equipment or caregiving roles for our patients. Sometimes, Social Workers help coordinate alternative living situations such as nursing homes or assisted living should home not be safe.

Case Managers coordinate patient care so that it is safe, timely, efficient, and client-centered. Our team follows each patient from admission to discharge, ensuring that all the details are accounted for each step of the way. Therapists can focus on speech articulation, voice quality, aphasia, cognition, language, food choices, and swallowing techniques.

Our Occupational Therapists help patients regain the ability to perform daily living tasks and everyday work activities following injury, surgery, or illness. Sometimes these treatments involve coping with new levels of ability. Other times, Occupational Therapy involves education and training to manage temporary disability.

WHY DO YOU LOVE HOME HEALTH?
"The patient impacted me the most was a young woman who, with her husband, had moved away from their hometown and extended family. Her new home was near shops and restaurants, but she was no longer able to get out on her own. She had spent much of the past year alone and in bed, as her husband worked during the day and she could not leave the house without him. It was the end of a long day for me and I was preoccupied with my own worry as I began the evaluation process. Together, we strategized how she was going to reclaim her independence. Together, we made a plan for her physical health as well as her emotional and spiritual well-being. We laughed, we cried, we hugged when I left. The gift she gave me was this: Though times may be tough, it is the connections we build with each other that matter most. Together we found hope.”
-Dorre graduated from Loma Linda University in 1993. She has worked in home health for four years.
