Endodontic treatment of tooth no 20 through a crown in one visit. An apical MTA plug was placed followed by gp/sealer. Two years post-op show pa and lateral lucencies healed completely. Patient has no symptoms. Large buccal parulis absent and tooth in full function.
Endodontic treatment of tooth no 8 in one visit. Placed an MTA plug followed by gp/sealer. Eight months post-op shows PARL healed, patient has no symptoms and tooth is in full function.
“Looks” don’t matter. “Beautiful white lines” on an xray don’t matter. A few of my not so “perfect looking” endodontics with long term post-op recalls.
The tale of an endodontic retreatment of tooth no 14 through a pfm crown, in one visit with a 6 months post-op. The root canal was done originally in Azerbaijan. Patient continued to have pain after the root canal that was dismissed by the Azerbaijani dentist as being “normal healing pain.” When patient came back to the USA she sought the help of an endodontist who took a cbct & recommended endodontic retreatment. Tooth was retreated by the said endodontist but patient continued to experience pain. Patient went back to the endodontist multiple times. Another cbct was taken by the said endodontist & she was recommended an apicoectomy. The patient declined & decided to get a second opinion. That is when she came to see us with her multiple cbct data on a flash drive. Watch the full video for the rest of this epic tale.

RCT of tooth no 2 with two years post-op showing it to be disease-free, asymptomatic & functional.

Endodontic retreatment of tooth no 14 in one visit. According to the patient RCT was done originally by patient’s general dentist. When the infection persisted her dentist referred her to an endodontist for endodontic retreatment. The endodontist retreated the tooth but the infection and symptoms persisted yet again. Patient was then referred to an oral surgeon who did apico on mesial root. After apico tooth felt ok for a few months but then symptoms returned yet again. Patient was then referred to us for evaluation. On exam, tooth no 14 had a crown and the endo access through the crown was restored with composite. It was tender to percussion and palpation. Probing was 2-4mm. Radiographs showed PARL. The apico’d mesial root did not seem to had been obturated with MTA by the endodontist in preparation for an apico or retrofilled with MTA or other suitable material by the oral surgeon. Discussed options with the patient. Patient decided to try to see if we could redo the RCT and save the tooth for her. Endo access made through the previous endo access. One build up removed noted a large perforation right in the center of the tooth. Also located a missed MB2 right away. Gutta percha removed using chloroform. ss k files and flare files. Reciprocation. Copious irrigation with 6% hypo. Sonic and ultrasonic activation. MB1 and MB2 had separate apical exits. Obturated the MB2 canal with gp/AH Plus sealer. MB1 in its entirety obturated with MTA. Repaired the perforation with MTA. Tooth temporized and patient referred back to her dentist. We will see the patient in 6 months for a post-op evaluation.
RCT of a calcified tooth no 18 in one visit. Patient also had a limited opening. Twenty four millimeters long roots. ss k files. 6% hypo. Sonic and ultrasonic activation. Warm vertical obturation with gp/AH Plus sealer.
