As A Positive Choice continued to grow, we found it necessary to make organizational changes. A Positive Choice and Serene Health have reached an agreement to merge and restructure the organizations in order to devote time, resources, and expertise to the Serene Health Enhanced Care Management and Community Support Program. These changes are beneficial for all employees and members as well, as new opportunities for advancement and programs to assist with member needs.

At Serene Health we will offer 5 of the pre-approved Community Support Services:

Housing Transition Navigation Services

Housing Tenancy and Sustaining Services

Housing Deposits

Short-Term Post-Hospitalization housing

Community Transition Services/Nursing Facility Transition to a Home

Job description
The Placement Coordinator is the first point of contact for patients and families seeking assistance for retirement living placement. Placement Coordinators are knowledgeable of housing resources and eldercare options available. The Placement Coordinator qualifies patients and families by providing an explanation of services and an intake consultation. The Placement Coordinator assigns patients and families to the appropriate Account Representative. The Placement Coordinator works closely with the Account Representative and Director of Referral to ensure patients and families are provided with the resources and referrals necessary to live an independent lifestyle within their community.

Core Responsibilities:

Receive initial inbound calls from patients seeking elder care and housing options.
Provide the initial intake for patients and families looking for elder care and housing options.
Provide clients with a pre-screening assessment
Assist the Account Representative with connecting customers to retirement living, elder care options, health care and housing resources.
Qualify patients who are contacting APC due to the need for resources and referrals
Contact and follow up with patients regularly
Educate patients and families over the phone by providing information, answering questions, and explaining their options.
Direct patients and families to the resources that may assist with their needs.
Attend training
Daily communication with prospective patients.
Daily communication with referral sources
Daily data entry
Contacting patients and families through an outreach program for outbound calls and data entry.
Keeping referrals logs and sheet updates daily
Preparing patient files with all appropriate documents and data entry.
Other duties as assigned
Experience:

1 year of customer service and sales experience, including responsibility for data entry

Wide and deep knowledge of Microsoft and Google
Proven leadership skills with the ability to motivate and inspire peers and customers
Strong interpersonal skills and the ability to add value to any project/initiative
Has a track record of introducing innovative ideas and solutions into business
Strong organizational and planning skills
Demonstrable skills in sales and customer service
Job Type: Full-time
