If you are an employed physician, I strongly urge you to go solo and take control over how you practice medicine. There is nothing more satisfying than being your own boss and practicing how medicine was meant to be practiced (aside from insurance issues). There are far less middlemen, administrative control over schedule or your practice. You set-up your practice and schedule as you see fit.
As with anything there are growing pains, a steep learning curve. But I wished I could've started sooner.

1-Regain control🚘
If you're not 🚫doing something about a situation it doesn't do any good to keep talking about it.
Harness that 🌊energy and analyze how to change the situation. You cannot⛔ control others around you or a situation. But you can choose how to respond, react or change the situation. If not feasible learn to adapt to keep stress levels low.
For example, you dislike⛔ your job, workload is crazy and you have no 🚫say. Why stay in it? Look for something else or open your own business📈. Life is far too short to be in a situation you dislike.
2- Let go of toxic people
You cannot🚫 control other people or their behavior. You do control your reactions and you control whether you can 🚶♀️walk away. No need to engage.
For instance, you know someone just constantly brings you ⬇️down. There's no need to interact with that person, if necessary keep contact brief but do not engage. Or remove🚫 yourself from the situation-move away, ask to get transferred, limit contact. Many ways to do this.
3-Let go of toxic 🚫habits
If you know it's bad for you why do it. In the end it does you no⛔ good. You can get satisfaction short-term but doesn't help you long-term and any positive goals long-term.
For instance- you know you have heartburn but still eat spicy 🧆food. Why go there. You know you had a heart🫀attack but continue to stay sedentary, smoke and eat fatty🥓 food. You do not help the situation
You have more 💪power than you know. You control your 🧠thoughts, reactions😲 and actions.
Make 2022 an empowering year

Just saw this amazing display, 250,000 lights on Saddle Creek. Saddle Creek is off Clark Rd. just across the Sarasota National Cemetery-drive all the way in he neighborhood for about 1/2 mile it's on the left side you can't miss it. Check it out! The owner said her husband starts 2 months before you can walk through it. Starts 6-10pm. Video came out blurry so I added clearer footage in comments.. Thanks to Dr. & Mrs. Dunn for this spectacular display. This should be on the news

Exercise is good for you.. I've always wanted to do kickboxing.
I had my first class.
Had a great workout at with Michele.
It has some overlap with Tae Kwon Do and Boxing. I had to pace myself because it's been awhile since I did a class in the martial arts category which is a very physical sport. I remember some of the kicks from my old Tae Kwon Do classes I used to do and some of the moves from a boxing class I once did. But I've never done kickboxing it looks like a combination of both. I remember being so sore in my arms a day later after an intense boxing class so I didn't exert myself as much today
Maybe next week I'll start giving a little more punch. It's great because we have one punching bag each.. It''s hard I found when I practiced kicks in the air when I used to do Tae Bo because then your force in the foot strains your joints. The punching bags absorbed all the force which is great.
We did:
-front kicks
-side kicks
-roundhouse kicks
For boxing we did:
-jabs
-cross hits
-uppercuts
-hooks
Also did some core exercises
I had lots of fun. My heart was racing.
I need to work on my push ups but it's all good.

occur because tiny little arteries can get clogged up die to the inflammatory process itself causing strokes. By the same token, when pathways aren't working one can have problems thinking called encephalopathy.
Sometimes the pathway is affected causing patients to form clots which move up to the blood vessels in the brain.
Some patients may have coming from areas of the brain that don't work right.
Patients can develop nerve problems.
Patients can have due to involvement of the arteries themselves.
These are only a few but most common.
A neurologist is trained in recognizing neurological complications and in treating patients.

Sometimes diffusion tensor imaging can pick up abnormalities. DTI can pick up abnormalities in white matter tracts...

Migraine treatment is not just about medicine...
If you have triggers in your lifestyle even the best medications will not work...I’ve said it before I’ll say it again because reiteration is always a good thing...
Simple modifications are found to be helpful:
1-stop all caffeine
2-avoid stress
3-hydrate
4-sleep continuously
5-eat 3x a day
such as Riboflavin and Magnesium oxide can help prevent if used daily... 50% of migraineurs are magnesium deficient. has anti-inflammatory properties...

Temporal lobe epilepsy
Hi, this is Dr. Thornley your friendly neighborhood neurologist. Temporal lobe epilepsy can comprise multiple types of symptoms because of its proximity to the insular cortex.
The insular cortex commonly is connected to the function of emotion and other types of sensations. It also impacts cognition
There are many symptoms of temporal epilepsy:
- anxiety
- fear
- having a funny indescribable sensation
- metallic taste in the mouth
- smelling foul odors - epigastric rise in your stomach
-loss of awareness
These are only a few of the symptoms that are manifested in Temporal Lobe Epilepsy.

I discuss temporal lobe epilepsy including localization-related epilepsy in the temporal lobe....
The temporal lobes are located on both sides of the brain. On the left side, in the superior gyrus there is the Wernicke’s area which is involved with comprehension in language. In addition, there is the Heschl’s gyrus which subserves the function of hearing. The temporal lobe is also responsible for the majority of our memory function as well as emotions. Within the temporal lobe is your hippocampus and amygdala. The hippocampus is involved with memory as well as emotions. The amygdala subserves the function of emotion...it has a memory component as well...
Localization-related epilepsy in the temporal lobe may be related to acquired conditions including head injury, infection, stroke, tumors, inflammation or other causes. It can also be related to congenital disorders...
Temporal lobe epilepsy pertains to a condition within the temporal lobe where there is abnormal discharging of the neurons causing seizures. Normally, there are electrical impulses which occur. Then the cells will eventually be inhibited. However, if there’s no inhibition then the firing continues until there’s synchronization. This results in an electrical signal-an epileptogenic spike or sharp wave on EEG (the brain wave test). The clinical manifestation of a seizure is dependent on the area involved....

Benefits of CBD in patients with seizures

I incorporate cannabinoids extensively in my practice. For the medical community looking for evidence, there has been research showing benefit of CBD in epilepsy (Devinsky, New England Journal of Medicine, 2017). Because it’s a plant, the medical community has been slow to adopt CBD as part of the armamentarium for treatment of seizures. Clinically, it does wonders.
A higher ratio of cbd or cannabidiol is far better for seizure control compared to a 1:1 ratio or lower ratios of CBD from dispensaries.
The over-the-counter full spectrum CBD seems to have some value as well in practice.
Definition of terms:
1-full-spectrum CBD-there is a legal amount of tetrahydrocannabinol of 0.3%. Taking the flower as a whole with all components work far better than an isolate. This is called the entourage effect
2-CBD isolate-is when CBD is isolated, far less effective because only CBD by itself is used here
3-CBD broad spectrum-THC is taken out but has the rest of the plant including terpenes and flavonoids. There are no potential side effects of psychoactivity because THC is absent.
