When you have a large cystic nodule in the thyroid gland, before considering surgery, consier Ethanol ablation (EA).
This simple procedure will reduce the nodule volume up to 90 % and it usually is permanent in reducing the volume of a cystic nodule.
I performed a procedure in this patient in Jan 2023 and in 4 weeks her nodule volume shrank by 77%.
Pressure symptom is gone and cosmetic concern also is gone.
Procedure took about 10 minutes in office vs. surgery (you will lose half of your thyroid out and you will likely end up on thyroid medication for the rest of your life).

1. Bioidentical is a business word not a medical word. Infact FDA approved HRT is BODY identical same as what your ovaries make.
2. Benefits of HRT in rightly chosen patients far outweigh the risks.
3. Estrogen decreases breast cancer risk.
4. Alcohol doubles the breast cancer risk
5. Compounded HRT including pellets when investigated have different contents in 35 percent of cases whereas FDA approved more affordable medication contained exactly what the ingredient list said it contained.


Be smart about your HRT

Reaffirming metabolic memory that was discovered in DCCT and UKPDS. In patients who were just diagnosed with type 1 diabetes, I highly recommend going on continuous glucose monitoring (CGMs) early in the course so you can achieve lifelong better glycemic control.

This graph shows the top no sensor, middle (no sensor then a sensor after 2.5 years of no sensor) then sensor initiated at the time of diagnosis.
Patients who got on CGM (sensor) after 2.5 years or so never achieve quite glycemic control that the early CGM adoptors were able to achieve.

Metabolic memory is real and repeatedly shown in many clinical trials.

First patient gains insulin independence with NOVEL Stem Cell derived islet cell therapy.

A man with long standing type 1 diabetes and no insulin producing function achieved insulin INDEPENDENCE after a SINGLE infusion of an investigational stem cell derived pancreatic islet therapy.

Thyroid and estrogen or SERMs (selective estrogen receptor modulators), aka tamoxifen, raloxifen, etc.

Estrogen can increase thyroid binding globulin which will lower bioavailable form of thyroid hormone.

SERMs are compounds that act as modulators can attach to the estrogen receptor to change its activity. SERMS (selective estrogen receptor modulators) are modulators for some estrogen receptors- turning up or down the effects of estrogen through binding to its receptor. SERMS have the same effect on thyroid hormone replacement, meaning it will increase thyroid hormone requirement.

On the other hand, AI (aromatase inhibitors) such as Femara, Arimidex, Aromasin can lower thyroid hormone requirment because of lowering estrogen level in the body.

If you are experiencing menopausal transition, or starting estrogen, SERMs, or AIs, you should check your thyroid levels to ensure your dose is still good.

Why Strontium is not advised for osteoporosis.

Many patients are lured by the advertised benefits of strontium for bone health. Strontium has a chemical similarity to calcium and can replace calcium in the bone.

Strontium ranelate- was used in Europe until 2012 and taken off of market due to significant increases in heart attack, blood clots, cognitive decline, seizure, liver inflammation, and reduced numbers of red blood cells.
Strontium results in overestimation of increase in bone density using DXA methods.

Strontium - likely to make future bone density test inaccurates (may reside in the bone for about 10 years after the last dose),
strontium citrate and strontium chloride are supplements and are not tested or regulated by FDA.

When you are under a superb endocrine care, this is what can happen. This patient came to me in spring of 2021. Danger of high fructose corn syrup.
This is as toxic to the liver as alcohol.
Yet children drink it and eat it daily.
For example, this Yakult, dubbed as healthy food, contains high fructose corn syrup.
This will cause diabetes, liver failure, cirrhosis to name a few.
Please check the ingredient list on your food you purchase and put it back on the shelf if it contains HIGH FRUCTOSE CORN SYRUP.

Many 'adrenal support' supplements sold online as energy boosters may contain thyroid hormones and steroids that aren't listed on the labels and can cause serious side effects.

Researchers purchased 12 most popular dietary supplements for enhancing metabolism, promoting energy, easing 'adrenal fatigue' the disease that does not exist, or providing adrenal support.

Lab test of the supplements found they ALL contained thyroid hormone and most had at least one steroid hormone.

'Natural', 'organic', 'herbal', 'plant based' may not be safe.

FDA does not regulate supplements and you need to be careful of the ingredients.

Always talk to your doctor before you start taking supplements!

If your doctor recommends a biopsy on thyroid nodule, make sure you ask your doctor if he/she has reviewed the images themselves. Not just a report but the actual pictures. For example, this spongiform nodule (mimicks kitchen sponge) does NOT need a biopsy since the rate of malignancy is less than 1%. We, board certified endocrinologist do NOT recommend biopsy. However, if the doctor does not review images, and simply rely on the report, they may recommend a biopsy. I have seen this happen to many of my patients who were under the care of endocrinologists even. Simply some doctors are too busy to look.

when you are recommended to have a needle stuck on your neck, make sure you do need that procedure.

Obesity is a chronic disease that worsens over time.

Weight loss does NOT cure obesity- it triggers adaptations that restore the lost weight. Body fights! Pharmacotherapy has been approved by FDA for CHRONIC weight management. This is a big paradigm shift. Prior to 2012, most meds were indicated only for short term use to induce weight loss. We know this does not work.

Pharmacotherapy has been shown to be both safe and effective. It increases the oddds of losing 5-10% of body weight or more x 2-3, and the odds of keeping that weight off. Unfortunately, weight is regained if treatment is stopped.

We need to educate other physicians who think patients who seek treatment for weight loss are lazy and are seeking quick fix. Some of them are but many of them are NOT and genuinely need help.
