Did you know that sciatica pain is a common condition that affects many people?

Sciatica pain typically occurs when the sciatic nerve, which runs from the lower back down to the legs, becomes irritated or compressed. Here are some common causes to be aware of:

1️⃣ Herniated Disc: When the soft, gel-like center of a spinal disc protrudes and puts pressure on the sciatic nerve, it can lead to pain and discomfort.

2️⃣ Spinal Stenosis: This condition occurs when the spinal canal narrows, putting pressure on the nerves, including the sciatic nerve. It’s often associated with age-related changes in the spine.

3️⃣ Piriformis Syndrome: The piriformis muscle, located deep in the buttock, can sometimes become tight or irritated, leading to sciatic nerve compression and pain.

💧Ketamine infusion therapy is an FDA-approved, non-opiate medication that is used to treat a wide number of medical conditions ranging from chronic pain to depression to tapering opioid dependency.

💥In the brain, excitatory neurotransmitters and NMDA receptors allow us to feel pain, and increased levels of activity in NMDA receptors leads to heightened sensitivity to pain. They play a large role in multiple chronic pain conditions including neuropathy and fibromyalgia.

🩷 October is Breast Cancer Awareness Month. More than 240,000 cases are diagnosed every year in the U.S. Chronic pain can be a result of cancer itself from putting pressure on nerves, organs, and bones as well as destroying tissue. Patients can also experience different types of chronic pain from cancer treatments.

🩺 Procedures such as a lumpectomy, mastectomy, or breast reconstruction can damage sensory nerve groups. Hormonal treatments including aromatase inhibitors (AIs) can often lead to musculoskeletal pain in the joints and muscles; and chemotherapy can result in peripheral neuropathy in the extremities.

💉 Nerve blocks and joint injections are one solution that can help ease cancer-related chronic pain conditions by using anesthetic medicine to reduce the transmission of pain signals.

💙 Dr. Aileen Padilla joined Comprehensive Centers for Pain Management in October 2023. She started her career in Fort Myers, Florida, where she completed more than 130 Minimally Invasive Lumbar Decompression (MILD) procedures and over 50 Intracept procedures.

💙 Dr. Padilla earned her Bachelor of Science in Biochemistry at the Iona College in 2009, and later went on to receive her Doctor of Osteopathic Medicine degree from the NYIT College of Osteopathic Medicine in 2015.

💙 She completed her residency training in Physical Medicine and Rehabilitation at the SUNY Health Science Center in Brooklyn, New York in 2019. That following year, she also finished an Interventional Pain Management Fellowship at the Center for Advanced Pain Management and Rehabilitation in Metuchen, New Jersey. Dr. Padilla also contributed to several medical publications, and was recently a panel speaker at the ASPN 2023 conference.

🔴 When a person gets sick or injured, the body has an acute inflammatory response. Pro-inflammatory compounds are responsible for reactions like swelling, redness, and bruising. This is an essential part of the healing process and a way to prevent further harm.

🔴 However, when the body experiences chronic inflammation, it continues to send out white blood cells even if there is no infection or injury present. Having too many pro-inflammatory compounds can throw the immune system into overdrive and cause harm to healthy cells. Left untreated, chronic inflammation can lead to autoimmune diseases such as multiple sclerosis, lupus, or rheumatoid arthritis.

🔴 Symptoms of chronic inflammation can often go undetected for long periods of time since physical signs like redness, swelling, and tenderness aren't present. How can chronic inflammation be prevented? Maintaining a proper diet and keeping up with regular exercise, and getting enough sleep can help protect against chronic inflammation.

How does a strong core prevent chronic pain?

💪 Strong core muscles maintain balance and stability, transfer stress through the muscles rather than the spine, and can prevent degenerative issues like spinal stenosis, herniated discs, or radiculopathy by keeping the spine properly aligned.

🔷 The core makes up nearly half of the body and includes all muscles that surround the pelvis and spine. If muscles around the lower back are weak, the body will rely more on ligaments and bone structures for support, which can lead to back pain, poor posture, and increase the chance of injuries.

What is Student's elbow?

👉 Olecranon bursitis, also known as 'student's elbow,' is a type of bursitis that happens when bursae in the tip of the elbow become swollen and inflamed. Bursae are small sacs of fluid that help lubricate, reduce friction, and absorb pressure between joints as the body moves.

👉 Student's elbow can be caused by putting too much pressure on a bursa like resting the elbow on a hard surface for long periods of time. This condition is also frequent among athletes who experience trauma from falling and hitting the elbow on a hard surface.

👉 A few common symptoms of student's elbow include swelling at the back of the elbow, feeling pain when moving the arm, or having a limited range of motion due to pain.

➡️ The sacroiliac joints connect the spine to the hips and provide support and stability to the body during movements such as walking, lifting, or climbing the stairs.

🦴 Osteoarthritis, accident injuries, and pregnancy labors can cause the SI joint ligaments to become stiff and weakened resulting in the sacrum and ilium bones grinding together. Symptoms of SI joint pain start in the lower back and pain and numbness may radiate through the lower hip and upper thigh.

🦴 An SI joint fusion procedure uses an implant to help restabilize the spine and hips. The implant has grooves and a cylindrical shape to secure it in place, and is made of cancellous bone material to stimulate cell growth between the sacrum and ilium bones.

👋 Many pain conditions associated with the wrist and hands are caused by repetitive strain injuries, or RSI. Repetitive motions and overuse from things like scrolling on a phone, typing on a computer, and even poor posture can result in inflammation or degeneration of the tendons in the hands.

👉 Carpel tunnel syndrome, tendinitis, and trigger finger/trigger thumb are a few common RSIs in the hand. These types of injuries gradually develop over time and different RSI can have similar symptoms including a dull aching pain, tingling or numbness, and stiffness or weakness in the joints.

What's the difference between trigger point injections, nerve blocks, and epidural steroid injections?

📍Trigger point injections are used to release trigger points, more commonly known as knots, that are caused by myofascial pain syndrome. Knots form when a group of muscle fibers stay contracted for long periods of time. This can prevent blood flow to the tissue and allow toxins to build up. Medicines like corticosteroid, anti-inflammatory steroids, or botulinum toxins (BOTOX) help reduce muscle tension.

💧Nerve blocks are a short-term solution to treat a wide variety of acute and chronic pain conditions where nerves are damaged, inflamed, or irritated. An anesthetic medicine is used to temporarily reduce the transmission of pain signals in a specific area. Nerve blocks are also used as a diagnostic test to locate the source of pain for procedures like radiofrequency ablation.

💉Epidural steroid injections are similar to nerve blocks in that both use local anesthetics and steroids to block pain signals. However, an epidural steroid injection specifically targets nerves in the epidural space, which covers a broader area from the lower back, hips, and legs. Multiple pain conditions such as a herniated disc, cervical radiculopathy, spinal stenosis, or sciatica can be treated at the same time.
