• Manage your A1C, blood pressure & cholesterol levels.
• Seek additional support for your specific needs.
• Make physical activity part of your routine.
• Follow a diabetes mean plan.
• Stay on top of your vaccines.

According to the Centers for Disease Control and Prevention, nearly 9 out of 10 Americans have limited health literacy skills and have difficulty understanding health-related information.
Low health literacy is associated with patients who are older, have limited education, lower income, chronic conditions, and those who are non-native English speakers.
Insufficient understanding of health information significantly impacts health outcomes.
The Serenity Life Center will offer services and programming to assess and address the health literacy needs of our community.
