May is Lyme Disease Awareness Month - Let’s talk prevention!
Over the past 10 years, I’ve had the opportunity to care for thousands of patients with Lyme and other tickborne diseases. I’m grateful to have been trained under some of the leading tickborne experts in both the US and Europe, and to be able to offer patients effective treatments. Thankfully, the majority of my patients have responded to treatment. Unfortunately, some still live with chronic symptoms, experience relapses, or have to reduce their daily activities due to these potentially devastating infections.
Let’s Talk Prevention:
Minnesota and Wisconsin are indeed “God’s country,” with absolutely gorgeous natural beauty to be enjoyed by all. Yet both states are endemic with Lyme and other tickborne diseases. We certainly cannot prevent all cases, but there are some steps we can take to drastically reduce our risk of contracting these infections while still enjoying the beautiful outdoors.
Permethrin:
One the most effective strategies when in the outdoors is to protect your clothing, gear, and shoes with permethrin. It’s a personal favorite of mine. You can also purchase clothing that has been pre-treated with the repellent permethrin at outdoor recreation stores and online at Insect Shield (protection lasts through 70 washings). You can also purchase permethrin spray from Insect Shield here: https://www.insectshield.com/products/permethrin-spray

Prophylactic treatment following a blacklegged tick (i.e. deer tick) bite:
If you are bitten by a blacklegged tick, prophylactic treatment can be started. While it is still common practice for some physicians to prescribe a single dose or a few doses of doxycycline for prophylaxis, this is not recommended and is NOT consistent with ILADS guidelines. Instead, ILADS, based on research in animal models, recommends that known blacklegged tick bites be treated with 20 days of doxycycline (barring any contraindications).
Acute Lyme treatment:
If any symptoms of Lyme occur following a tick bite, it is important to treat promptly and for a long enough period of time. This drastically reduces the risk of developing chronic Lyme. Given the low success rates in trials treating EM (bull’s eye) rashes for 20 or fewer days, ILADS recommends that patients receive 4-6 weeks of doxycycline, amoxicillin or cefuroxime.

Microbes and Mental Illness

Did you know that many mental disorders, including anxiety disorders, depressive disorders, autism, schizophrenia, as well as suicidality and violent behaviors, can be associated with certain microbes? One of my favorite colleagues to hear from, Robert Bransfield, MD, a professor of psychiatry at Rutgers, has been a thought leader in how infectious disease can play profound roles in the development of neuropsychiatric issues. I regularly see Dr. Bransfield at medical conferences regarding Lyme and tick-borne diseases, as well as at medical conferences regarding autism. Last month, Dr. Bransfield published a fantastic paper, entitled, Microbes and Mental Illness: Past, Present, and Future in the medical journal, Healthcare. While you may not have time to read this entire scientific article, I'd like to comment on some of the content that is relevant to many of my patients.
Twenty years ago I was finishing my premed studies at the University of Minnesota and I decided to minor in the history of medicine. Historically, most diseases, including mental illness, were very poorly understood. Mental illness was often thought to be caused by an angry god or demonic possession or witchcraft. In fact, the earliest evidence of the recognition of mental illness was the discovery of trephination, in which holes are drilled into a skull, most likely to relieve mental illness or headaches, thought to be caused by evil spirits.
During the 1800s, several scientists, including Louis Pasteur, were influential in the development of germ theory and how microbes could cause disease. This led to significant advances in medicine.
The most classic example of a bug causing psychiatric disease is late-stage syphilis, a disease caused by Treponema pallidum. In late neurosyphilis, general paresis (also known as general paralysis of the insane), causes neuropsychiatric symptoms ranging from cognitive impairment to psychosis.
I do want to draw attention to this paragraph, regarding Lyme and psychiatric manifestations:
Lyme borreliosis/tick-borne disease is associated with almost any psychiatric syndrome listed in the American Psychiatric Association DSM5-TR. The neuropsychiatric symptoms seen with Lyme/tick-borne disease include developmental disorders, autism spectrum disorders, schizoaffective disorders, bipolar disorder, depression, anxiety disorders (panic disorder, social anxiety disorder, generalized anxiety disorder), posttraumatic stress disorder, eating disorders, sleep disorders, addiction, su***de, violence, anhedonia, depersonalization, dissociative episodes, derealization, hallucinations, intrusive symptoms, and vegetative functioning impairments. Possible cognitive impairments that can be precipitated include dementia and impairments in attention span, memory, processing, and executive functioning.
And treatment can make a difference!
“In 75% of chronic Lyme disease patients with neurocognitive and/or mood dysfunction
that impaired their daily living activities, single-photon emission computed tomography
brain imaging demonstrated perfusion deficits to various areas of the brain, most
notably the frontal, temporal, and parietal lobes. Patients considered to be seropositive
and those considered seronegative had similar rates and severity of perfusion defects.
Antibiotic treatment, especially agents with intracellular-penetrating activity, resulted in
the resolution or improvement of abnormalities in 70% of patients over a 1- to 2-year
period.”

Do you know your GGT level? Elevated GGT is correlated with an increased risk of death from any cause! Testing your GGT level is simple and relatively cheap ($19), and can help you to optimize your health.

GGT, or gamma-glutamyl transferase, is an enzyme that's involved in recycling the very critical antioxidant, glutathione. As our GGT increases, so does our risk of death from any cause.
Let’s delve into the details on why this simple blood test is so crucial.
In routine medical care, the GGT level is generally only used to screen for alcoholism. This is because excessive alcohol consumption will deplete glutathione, thereby raising GGT levels. However, the antioxidant glutathione is essential for the detoxification of any toxin. Any toxin - including alcohol - will deplete glutathione, which causes an increase in GGT levels.
Much of the population has GGT levels that are well within normal range, yet the research suggests that these “normal” levels can still be linked to increased risk of death from any cause. For example, men with a GGT of 40-50 (within normal range) have a 20 fold increased risk of developing diabetes! Similar trends with GGT have also been shown to increase risk of cancer, cardiovascular disease, respiratory disease, liver disease, and high blood pressure.
Simply put, GGT is upregulated (increased) when the body’s demand for glutathione is increased.
So, what can we do about it? How can we lower our GGT (and raise our glutathione levels)?
Limit our consumption of alcohol.
Limit our exposure to other toxins, such as “persistent organic pollutants” (POPs), found on conventionally grown foods. (A good resource is the 2023 Shopper’s Guide - Dirty Dozen, from Environmental Working Group - When choosing produce, this is a helpful resource which identifies which products contain the most pesticides. For the most chemically-laden fruits and vegetables on this list, avoid, or opt for the organically grown versions instead. This is a topic for another day, but while USDA certified organic food is not necessarily any more nutritious, the organic growing process limits exposure to toxic pesticides and herbicides.)
Supplement with N-acetyl cysteine (NAC). Cysteine is critical in the production of glutathione, in fact it’s the rate-limiting factor. 1000 mg/day of NAC will increase glutathione levels in nearly everybody.
Administer high quality glutathione - orally, topically, intravenously, or intranasally.
So, please be sure to know your GGT level. Request it to be checked at your next visit to Northern Lights Medicine or at your next visit with your primary care physician. GGT is NOT expensive to check. A GGT at our clinic only costs $19.
