Arterial catheters (ACs) are vital for haemodynamic monitoring and blood sampling but often face complications. This study investigates the incidence and risk factors of AC failure in ICU patients.
Key Findings:
- 26% of patients experienced AC failure.
- Suspected catheter-associated bloodstream infection (CABSI) was the most common failure type.
- Ultrasound-assisted insertion significantly reduced failure rates.
- Increased age and certain demographics influenced failure risk.

Are we striking the right balance between reducing infection risks and minimizing waste in ICUs?
The latest study examines current guidelines for changing infusion sets connected to central venous catheters (CVCs) in Australian and New Zealand intensive care units (ICUs). Surprisingly, while recent evidence supports extending the replacement period to 7 days without increasing infection risks, only one-third of ICUs have adopted this practice.

How often are infusion sets for central venous catheters changed in Australian and New Zealand Intensive Care Units? A point prevalence survey - PubMed Most ICUs participating in this survey had policies to change their CVC infusion tubing in 3-4 days, and recent high-level evidence supports an update to extend this to 7 days. There remains work to be done to spread this evidence to ANZ ICUs and improve environmental sustainability initiatives.

Critical Findings on CVAD Failure
The recent study highlights that 10.2% of central venous access devices (CVADs) fail before therapy is complete. Major complications include CLABSI, occlusion, and dislodgement. Key risk factors are blood product administration through PICCs, fewer lumens in TCVADs, chemotherapy, and diabetes. Protective measures such as antimicrobial NTCVADs and lipid administration in TCVADs can help.
