Who would have thought that my intermittent bike riding challenges would have resulted in a RPM of a different kind. No, not spin class, but rather under the amazing directives of Peter Baines, founder of , one of the largest fund raising organisations in Australia. I found myself with 39 other persons from Australia with attributes of Resilience, Perserverance and Motivation ( RPM), cycling 520 km in 5 days across the rough terrain of the most northern villages of Thailand, battling the torrential rains of the wet season, with intermittent bursts of searing heat and humidity in order to raise money for the children of the orphanage, so that they can live their best lives. The inspiration was drawn from a Thai power woman and one of the few female monks, Mae Thiew, the Director of Baan Home Hug who has dedicated her life to the care and support of the most vulnerable children in Thailand. This was especially required during the most recent tsunami and she continues to battle challenges especially when limited by resources. There were times when the children would frequently die from preventable diseases as a result of a lack of access to medicine that would otherwise sustain their lives.
This Tour de Thailand was much tougher than most of us expected mostly due to the conditions but we persisted in trying to achieve our goal of raising money. We have currently achieved over $260,100. It was an amazing bonding experience and true friendships were made in just a very short but intense time.
I was also privileged and honoured to be asked by one of the founders and organisers of the ride Patrick Ng to be one of the keynote speakers and in doing so joined the prestigious group of presenters, leaders in their field including Peter Baines, Terry Portelli,
I cannot let it pass without a very special thank you to the backbone of the tour and without whom we would still be lost and wandering helplessly in countryside Thailand. She also helped co ordinate our fabulous support team, buying special and new delicacies from the road side markets for our pitt stops- ! We were so lucky to have you. Also special mention to the much loved , right hand to Terry, as well as the photography crew Maya and Wulan.
A super special mention MUST go to , the best roomie ever. From the moment I met her when I walked in late to our room she put up with me from getting an engineer to find a simple wall switch to lower our bathroom blind, to putting up with a mud caked shower when I walked in helmet, cleats and all to wash the Mekong off me after a day of hardly being able to see the huge puddles and pot holes due to the pouring rain. That takes courage and fortitude just to put up with me!! She is also now a powerful cyclist, so watch out Sydney!!
Lastly special thanks to the generous sponsors who also came on the ride and without whom this event would not have been so successful , OYOB Property, Indie Lifestyle and Raine & Horne Berwick. I left with such an elated feeling to know that the rest of the world is coming on board with the mission. It will take some time to implement a simple saliva DNA test into the regime of Preventative and Predictive health but it must happen as part of standard future healthcare. An exciting sneak peak into the future ….. watch this space.

Here I am attending the 7th World Congress on Controversies in Breast Cancer (CoBrCa).
It was great to participate in this unique scientific event – here’s me with my colleagues and event key speakers (back right is Prof Javier Cortes from Barcelona, Spain; back left is Prof Alister Thompson from Houston, Texas USA.)
CoBrCa, again, lived up to its promise, and was as exciting and informative as ever, directly addressing controversial topics in breast cancer management with key issues explored and audience participation encouraged. This multi-faceted congress of interest to all involved in the treatment of breast disease showcased many top local and international experts to the UAE. Through debates, discussions and presentations, the congress focussed on issues faced by clinicians in daily practice.

I am a staunch advocate for advanced medical solutions, and I valued the opportunity provided by Seven News to voice my support for the test, where I emphasize the paramount importance of having diagnostic tools that transcend the limitations of relying solely on family history.
With this innovative test, scientists can ascertain a patient’s likelihood of developing significant health conditions. Simon Morriss, CEO GeneType, an expert in the field, emphasized the transformative nature of this development, describing it as “one of the most amazing and exciting” advances that stands to redefine the future of preventative care.

“Ballarat’s Fiona Elsey Cancer Research Institute has just announced that renowned Melbourne specialist breast surgeon, Dr Nicole Yap, will work with the Breast cancer research group as a Collaborating Clinician in metropolitan Melbourne.”
(taken from media release dated June 2023)
As the first Metropolitan based breast surgeon to be accepted as a collaborating clinician, I am honoured to officially support this important research team and I encourage other breast cancer surgeons to follow in my footsteps to help bring more patient perspectives and expose the research to a wider audience.
I have chosen to donate my resources to the Fiona Elsey Cancer Research Institute based on their current studies which align with my charity Australian Breast Care Centre.
Triple negative breast cancer is one of the most aggressive breast cancers and tends to be associated with women under the age of 50 and certain genetic mutations.
We’ve come a long way from my last fundraising efforts at the Fiona Elsey Ballarat Cycle Classic in March this year where I got to meet the team and forge this exciting journey. Thank you Prof George Kannourakis for the opportunity to work with you and your amazing team to help find a cure.

Dr Nicole Yap Dr Nicole Yap is a Specalist Breast Surgeon whose interests lie in Breast Oncoplastic Surgery
Over the last year my charity Australian Breast Care Centre has received generous support from our valued sponsors who have provided prizes and funding to help us build awareness of breast cancer – especially in women under the age of 50, as well as preventative tools in identify risks associated with breast cancer.
In the last year we have given away over 150 free genetic test packages that include a consult with specialist breast surgeon, moi.
I’d like to share a couple of patient stories that really touched me and reminds me why I do what I do.
At an event where I was presenting, I met a young Jewish lady of Ashkenazi decent (who are well known for inherited mutations) and she had already tested BRCA-negative.
Following my presentation, she realised that 85% of all breast cancers have no gene mutation and approached me about the GeneType test which assesses the individuals risk for developing a breast cancer.
In total she has now had two tests, one for gene mutation, the other was for breast cancer risk assessment which assesses your actual risk of developing a breast cancer in the next five years, as well as your lifetime.
Although she tested negative, this has benefited her psychologically and has allowed us to develop an ongoing tailor-made breast care management plan.
Another patient story is about how risk assessment enabled her to detect an early breast cancer, which counteracted general advice and consequently saved her life…
I would also like to acknowledge and say thank you to those who have donated to our not for profit charity, resulting in some of you being lucky enough to win free genetic testing.
I previously interviewed my patients on the topic, you can view their first-hand stories on episode four of my educational series.

As a specialist breast surgeon I treat both the cancer and the cosmetic outcome, a field becoming more commonly known as oncoplastic surgery.
So, I would like to give kudos to Royal Australasian College of Surgeons for sharing a story recently featured in the The Australian. It’s reassuring to hear that the rate of breast reconstruction following mastectomy has increased 126% over the last 10 years.
As a specialist breast surgeon, I have the ability to remove and treat the breast cancer whilst maintaining or possibly improving the aesthetic/cosmetic outcome. With a dual trained surgeon, this procedure only requires development of a rapport with a single surgeon rather than two.
In my practice working in the field of oncoplastic surgery, I perform on average one case per week as a single surgeon, which involves the cancer surgery combined with reconstruction in the one operating session.
I have certainly seen positive outcomes in my patients, which include: reduced recurrence rate, reduced new primary breast cancers and improved phycological impact with improved breast aesthetics.
Whilst I appreciate this is a big step for patients diagnosed with breast cancer to take, the long-term outcome is certainly beneficial.
Whilst delayed reconstruction is more commonly available (due to two surgeons often required to perform both procedures) – the benefit of being dual trained and qualified to do both procures in one operation, results in a more timely, seamless and less traumatic experience for the patient.
This improvement in the approach to the management of breast cancer should enable patients to deal with this disease in a more positive way. A positive mind assists in achieving an overall improved outcome.
