The last instalment on Verbal Skills in the Verbal Assessment of a Patient, following the writings of Geoffrey Maitland.
Retaining control is the skill of allowing the patient to answer fully, and yet not to ramble or go off on a tangent that does not add to the assessment.
Using the patient’s words and language will aid in their understanding of what you are asking.
Speaking slowly and deliberately is more important now if using video or, especially, telephone consultations.
Keeping questions short avoids potentially confusing the patient (and the therapist!) and allows them to focus on answering the question asked.
For the same reason, ask only one question at a time: this also helps the therapist to know which question the patient’s answer belongs to!
Paralleling skills is knowing how long to let a patient follow a line of thought to ensure that the patient makes all the points they need to make without it becoming rambling. Unnecessary interruption may make the patient lose their place and forget to mention something important!
Monitor the patient’s facial expressions for signs they do not understand you! If so, repeat or restate your question with more clarity – simpler, using the patient’s words and/or avoiding technical terms.
Lastly, be aware of communication errors on both sides, and ask questions where needed to confirm that the patient understood your question, and that you understood their answer.

How many of us have a model for thinking for our assessments?
How many of us have thought about a plan for how we conduct our assessments?
Are we confident in our verbal assessment skills?
A͟r͟e͟ ͟a͟s͟s͟e͟s͟s͟m͟e͟n͟t͟ ͟s͟k͟i͟l͟l͟s͟ ͟s͟t͟i͟l͟l͟ ͟t͟a͟u͟g͟h͟t͟ ͟i͟n͟ ͟U͟n͟i͟s͟?͟
Or is this type of “critical thinking” only featured in post-Grad studies?
Now is a great opportunity to 𝗿𝗲𝗳𝗹𝗲𝗰𝘁 on our assessment skills to ensure they are efficient and effective. This is especially so if conducting 𝘃𝗶𝗱𝗲𝗼 𝗰𝗼𝗻𝘀𝘂𝗹𝘁𝗮𝘁𝗶𝗼𝗻𝘀, and even 𝗺𝗼𝗿𝗲𝘀𝗼 if conducting 𝗽𝗵𝗼𝗻𝗲 𝗰𝗼𝗻𝘀𝘂𝗹𝘁𝗮𝘁𝗶𝗼𝗻𝘀, where there is a greater reliance on the verbal assessment to extract information from the patient.
Does anyone practice their verbal assessments?
Maybe now is the time to 𝗿𝗲𝗳𝗹𝗲𝗰𝘁 𝗼𝗻, 𝗮𝗻𝗱 𝗵𝗼𝗻𝗲, your skills in this area.
The pivot to 𝘃𝗶𝗱𝗲𝗼 𝗰𝗼𝗻𝘀𝘂𝗹𝘁𝗮𝘁𝗶𝗼𝗻𝘀 shines a light on the assessment skills of Physiotherapists and others.
At this time, it is worth revisiting the many words written
by 𝗚𝗲𝗼𝗳𝗳𝗿𝗲𝘆 𝗠𝗮𝗶𝘁𝗹𝗮𝗻𝗱 on the topic of assessment.In particular, the 𝗽𝗵𝗿𝗮𝘀𝗶𝗻𝗴 𝗼𝗳 𝗾𝘂𝗲𝘀𝘁𝗶𝗼𝗻𝘀 and the confirmation
that the patient understood the question as was asked.
This necessitates some more thought into the wording and
phrasing of the questions, especially as the ability of the patient to take-in visual cues (body-language / non-verbal cues) can be hindered in video consultations.
A few points to consider:
1. Are you clear on 𝘄𝗵𝘆 you are asking that question and what information you
wish to gain from the patient as a response?
2. Does the 𝘄𝗼𝗿𝗱𝗶𝗻𝗴 of the question clearly ask what you
need to know?
3. Don't 𝗮𝘀𝘀𝘂𝗺𝗲 the meaning of the patient's words, enquire further to be certain.
4. Be careful that the patient's answer is not
𝗶𝗻𝘁𝗲𝗿𝗽𝗿𝗲𝘁𝗲𝗱 according to the Physiotherapist's experiences - again, enquire and clarify to be certain.
5. Make sure the patient answer 𝗿𝗲𝗹𝗮𝘁𝗲𝗱 to your
question and gave you the information you were after. If not, enquire further or your further assessment by contain errors.
6. Now plan your next question - are you 𝗿𝗲𝗮𝗱𝘆 𝘁𝗼 𝗺𝗼𝘃𝗲 𝗼𝗻 or do you need to explore the current point further?

This is where you give some of your injury and health history so that it can be determined if your situation is suitable for a Telehealth Consultation. If appropriate, a Telehealth Consultation can then be booked in.
Social distancing and self-isolation / quarantine has meant many people have their access to Physiotherapy Services limited. However, in many cases, this does not mean that the patient has to forego Physiotherapy treatment for their condition. Telehealth consultations still allow Physiotherapist's to use many of their skills to assess, diagnose, treat and monitor progress of patients. Patients can still get the benefits of Physiotherapy treatment letting the Physiotherapist and patient discuss how best to deliver the service.

Having to self-isolate doesn't mean having to put up with pain or injury, or missing out on your post-op rehabilitation.
You can now be assessed, diagnosed, reviewed and have a rehab plan started by the Physiotherapist using online / Videocall services or via telephone.
You should contact the clinic first to see if your situation is suitable for a Telehealth consultation.
Consultations are covered by Medicare if you are under a Chronic Disease Management plan and have been referred for Physiotherapy treatment.
Private Health Funds are currently not covering Telehealth consultations, however this is due to change on 14 April. Further details TBA.

SIRA / WorkCover / CTP Telehealth Consultations
Due to the potential for COVID-19 to impact a worker’s ability to access and providers’ ability to deliver face to face treatment services, SIRA is currently updating the relevant Fees Orders to expand telehealth services. This includes the delivery of consultations via telephone in addition to the already occurring video consultations. Consultations would be inclusive of any electronic communication to support the delivery of the treatment service. Telehealth services require pre-approval from the insurer and must be consented to by all parties – the worker, practitioner and insurer.

Several private health funds have agreed to provide benefits for individual (one on one) physiotherapy teleconsultations where:
• The customer is undergoing an existing course of treatment and the customer has seen the physiotherapist over the past six months, or
• For new patients, the tele-physiotherapy service has been recommended by their general practitioner or relevant medical specialist, and
• The primary condition being treated is one of:
Post orthopaedic surgery rehabilitation (e.g. Total hip or knee replacement)
Chronic musculoskeletal condition (e.g. osteoarthritis)
Cardiac rehabilitation
Pulmonary rehabilitation, or
Pelvic floor muscle training,
• The service is delivered before 30 September 2020, and
• The service is undertaken in accordance with Australian Physiotherapy Association guidelines.
Health funds will provide coverage for teleconsultations provided by physiotherapists from Tuesday 14 April 2020 subject to the conditions listed above.
