The hypothalamus is the size of an almond, and is a portion of the brain, which has an important function of linking the nervous system to the hormonal system via the pituitary gland.
Hypothalamic obesity can occur due to the following reasons:

1. Tumour in or near hypothalamus
2. Following removal of hypothalamic tumour
3. Radiotherapy

The most common tumor type associated with the hypothalamus is craniopharyngioma. Excessive weight gain is noted within three months of damage to the hypothalamus.
The weight gain with hypothalamic obesity is due to

1. Reduction in resting and voluntary energy expenditure
2. Multiple hormone deficiencies
3. Irregularities in insulin secretion
4. Growth Hormone deficiency
5. Hypothyroidism

There is no drug, which is specifically approved to treat people with hypothalamic obesity. Weight loss surgery may help this group of patients.

This is a genetic disorder in which the body fails to respond to parathyroid hormone (PTH). PTH produced by the parathyroid glands in the neck helps control calcium, phosphorus, and vitamin D levels in the blood and bone. In pseudohypoparathyroidism, the body produces the right amount of PTH, but is resistant to its effect causing low calcium and high phosphate levels in the blood.

Symptoms associated with pseudohypoparathyroidism include:

1. Cataracts
2. Dental problems
3. Tingling and numbness
4. Muscle cramps
5. Seizures
6. Excessive weight gain

Insulinoma is a small and usually non-cancerous tumour affecting the islet cells of the pancreas, which produce insulin. Insulinoma, may result in the overproduction of insulin and cause low blood sugar levels. Normally once we eat our blood sugar rises, signalling the pancreas to produce insulin. After the blood sugar level is back to normal, insulin production should stop. With insulinoma, the production of insulin continues and this can lead to dangerously low blood sugar levels or hypoglycaemia. Weight gain occurs in 20-40% of patients, as they have to eat frequently to prevent the symptoms of hypoglycaemia.

Other symptoms of insulinoma include double or blurred vision, mood swings, weakness, excessive hunger, sudden weight gain, occasionally weight loss, sweating and palpitations.

Checking blood sugar and insulin levels in the blood helps in diagnosis of insulinoma. A low blood sugar with a high insulin level confirms the diagnosis of insulinoma. Further scans including CT scan, MRI and octreotide scan can help localize the tumour. Surgical removal of the tumour is the treatment of choice for insulinoma.

Andropause is also known as male menopause, which most men encounter as they age. This usually starts after the age of 35 and is associated with reduction in testosterone production.

Men during andropause may experience physical and mental changes such as increased breast tissue, fatigue, low libido, reduced facial hair and mood swings. They also may experience erectile dysfunction, weight gain, depression, emotional changes, tiredness and sleep apnoea.

A blood test is the only way to diagnose a low testosterone level. Some men may not have symptoms even if they have lower than normal testosterone levels. If you have all the above symptoms, you should discuss with your Endocrinologist about the benefits of testosterone replacement. In some men testosterone deficiency may be due to causes other than aging, including problems with testicles, prostate or pituitary gland.

Mental health disorders are directly linked to physical conditions, yet only one of these is widely discussed in society. The other one is considered “taboo”, even though 1 in 6 people in the PAST WEEK will have experienced a mental health episode. Did you know that schizophrenia is associated with double the risk of death from heart disease and 3x the risk of death from respiratory disease?

Did you know that people with depression and schizophrenia are 2-3x more likely to smoke?

Also, did you know that people who are depressed or stressed are more likely to emotionally eat which leads to weight gain or obesity? Wellbeing and weight loss are connected. The mental and emotional weight needs to be unloaded before the physical weight can be successfully dealt with, since many underlying causes of weight gain stem from the mind.

All of these serious maladies arise from poor mental health...and we still feel embarrassed to talk about it? Please NEVER be ashamed of your illness. You cannot imagine the number of people you can inspire with your story, your strength and your resilience.
