Aesthetic crown lengthening - Upper 2-2➡️⁣
This young lady didn't like her gummy smile. She wanted to see if something could be done - but did NOT ❌want any composite bonding/veneers.

This was a challenging case where the teeth were crown lengthened using a combination of gingivoplasty and osteoplasty. This exposed more of her natural tooth structure, but CRUCIALLY did not expose any of the roots of her teeth.

The stunning result left her with ideally shaped gingival architecture and the uncovering of the full-length of her natural teeth, giving her a result she has been delighted with!
Her only complaint? She wished she'd had this done 10 years earlier....!

🦷Before🦷The patient was referred complaining of pain and swelling associated with her LR6 implant placed a few years earlier in Italy.⁣
There was pus draining from the area and a deep pocketing of 8mm

On examination, it became apparent that the implant had actually fractured - a relatively rare occurrence.

🦷After 🦷The failed implant had been removed and there is extensive loss of buccal bone, leaving a large defect.

The patient would like to have a new implant placed here after healing is complete.

Had the implant been left, bone loss would have been progressive, affecting future implant placement in this area.

Perception of "beauty" relies heavily on a symmetrical appearance - this lovely lady wasn't happy with the "funny gum appearance" of her front teeth. She was complaining "my front teeth are not the same height and they look crooked"

We’ve been seeing a recent increase in demand from patients like this lovely lady who want to “sort my gummy smile!”
This is a perfect example whereby Dento-alveolar compensation occurs due to excessive tooth wear (in this case as a result of both attrition and erosion). 🧐
Why does this happen? Because as the front teeth get more worn down, functional occlusion is maintained through modification of the dentoalveolar complex.
Function is maintained….but the aesthetics suffer! 😬
So….can we do anything about it??

Failing implants: What are the options?🦷

This is the case of a patient referred to Dr. Simon Stern, who had a failing implant.

In the case of a failing implant there are two options:

1. Remove the implant
2. Stabilise the implant

Most periodontists would have removed the implant, however, extraction is a painful and expensive treatment. The alternative is to stabilise the existing implant, even though the treatment could cause gum recession around the area.

After careful consideration, Dr. Simon Stern and the patient decided to stabilise the existing implant, saving the patient thousands of pounds and making for a much less painful procedure.

Dr. Simon Stern then successfully saved the existing implant, and now, three years later, it is still healthy and stable.🙌🙌

The patient was very pleased with the honest consultation that had saved him thousands of pounds and the healthy result of the treatment.🤩

How bad can a tongue piercing be? Can it impact my teeth and gums?👅🦷

YES! This is the case of a young woman who was referred to me suffering from severe gum recession….. affecting the inside aspect of her lower front teeth.🦷

She was suffering from acute sensitivity as a result of the recession - a large amount of the root of her tooth was now uncovered due to the trauma caused by her tongue piercing and this meant even something as benign as a glass of water would set the tooth off and be incredibly uncomfortable.😥

Worse than that….she couldn’t brush the area properly because it was so tender and she was getting gum inflammation on top - this was the perfect recipe for things to worsen and lose even more gum!

I see a lot of recession cases - but recession on the tongue-side region of the mouth as in this case, is particularly rare. And when it causes problems such as in this case, it needs to be treated.

So this was not just a rare problem, but a challenging one too. There is an important artery hiding down beneath the tissues here, just under the floor of the mouth, and access, as you can imagine, is limited.

Surgical intervention had to be performed in an incredibly precise and careful manner in order to cover some of the recession and, more importantly in fact, give the patient thicker gum in this area to stop the issue progressing and allow her to eat, drink and brush properly. 🪥🦷

Tissue was taken from the roof of her mouth and grafted onto the affected region once the site had been appropriately prepared. The patient was followed up carefully and the tissues healed up beautifully, giving the patient the thicker band of tissue she needs to keep them healthy in the long term.💪✨

She has been back to see me for regular reviews and has been delighted that her gums have been stable, and comfortable, ever since.😁

So if you’re thinking of getting a tongue piercing….think again!

Can a tongue ring cause gum disease? 👅

This is the case of a young lady who was about to lose her lower left front tooth due to an infection caused by her tongue ring.

This patient's tongue ring kept hitting her inner lower gum and eventually caused it to become infected. Resultantly, she developed severe gum disease which caused the loss of gum tissue and gum recession on her inner lower gum.😬

Her lower left front tooth was very unstable and was on the verge of falling out. Losing a tooth, especially a front tooth, is a frightening prospect for anyone.😰

The recession was on her inner lower gum, an area of the mouth which contains many arteries. Therefore, this case was both uncommon and surgically challenging.

Utilising the advanced medical technology of Bone Grafting, Dr. Simon Stern successfully treated her gum disease. By creating a graft, Dr Stern augmented the patient’s bone and secured the patient’s tooth to her gum.🦷✨

“Gummy smile”⁉️
Here is what we can do about it 🦷💫

This is the smile of a young lady who had been grinding her teeth as well as losing some tooth structure from erosion.

Over the years, as her teeth became worn down due to grinding, she felt that her gums became more and more prominent.

When she decided to undertake the treatment with Dr. Simon Stern she was just 36 years old and very self-conscious about her “gummy smile” which stopped her from smiling fully. 😬

Dr Stern proceeded with a dental aesthetic crown lengthening, which involves removing the excess gum tissue and re-contouring the gum line to expose more of the natural tooth. This young woman’s smile was affected by severe gum disease which had caused a gum recession.
With a naturally bright, beaming smile, she felt self-conscious about her gums and lacked the self-confidence that she once had.

Following an initial consultation with Dr Simon Stern at , she decided to proceed with treatment.
