Social Prescribing in Nottingham
WHAT IS SOCIAL PRESCRIBING?
Social prescribing connects people to practical and emotional community support, through social prescribing link workers, who are based in GP practices and take referrals from all local agencies. Link workers have time to build trusting relationships, start with what matters to the person, create a shared plan and introduce people to community support.
It helps people get more control over their healthcare, to manage their needs and in a way that suits them.
It can especially help people:
with one or more long-term conditions
who need support with their mental health
who are lonely or isolated
who have complex social needs which affect their wellbeing.
Social prescribing links individuals to a range of activities that are typically provided by voluntary and community sector organisations, for example, volunteering, arts activities, heritage activities, group learning, gardening, befriending, cookery, healthy eating advice and a range of sports.

PRACTICE BLOOD TESTS & HOME VISIT BLOOD REQUESTS

Aspley Medical Centre
509 Aspley Lane
Aspley
Nottingham
NG8 5RU
0115 929 2700

AN INFORMATION LEAFLET FOR PATIENTS REQUESTING A PRACTICE OR HOME VISIT BLOOD TEST IS AVAILABLE IN RECEPTION

PLEASE NOTE
As from 26th April 2024

We will no longer have the facility at the practice to provide both surgery blood tests and home visit bloods.

We will still be booking blood tests at the practice but will have limited capacity and you may be asked to have your blood test done at the hospital as this may be quicker.

If you meet the home visit criteria below, we will still be able to book a home visit to take your bloods but please be advised it may be quicker for someone to take you to the hospital if your carer/family member has transport available.

Our current phlebotomist will be leaving at the end of the month and therefore we will not have the capacity to undertake the amount of blood tests we have previously been able to do.

IF YOU THINK YOU MAY NEED A HOME VISIT

We would kindly ask that anyone who is mobile, uses walking aids, wheelchairs or scooters and are seeing or using other services away from home, come and see us in surgery.

If you are poorly and think you need a same day urgent visit, please ring your request through to reception on 0115 929 2700 as early in the morning as possible and before 12 noon. The doctor will always consider your request.

If we feel your visit request is inappropriate, we will ask you to use the service appropriately in the future.

Please do not be offended, as we have a duty of care to use our resources effectively for the safety of all our patients.

We have noticed that many patients are requesting home visits which are inappropriate or unnecessary and this can have a negative impact on our service. Calling for a home visit if you are able to attend the practice takes them away from patients who may have a more urgent clinical need. Most home consultations could easily and safely be carried out at the surgery and would safe crucial time and home

*A PATIENT WHO IS DEEMED TO BE HOUSEBOUND IF:-

Reasons for a Home Visit
APPROPRIATE UNSUITABLE
BEDBOUND

NO TRANSPORT OR MONEY

TERMINALLY ILL
YOUNG PEOPLE OR ANYONE WHO IS MOBILE

MAY COME TO SERIOUS HARM IF MOVED SOCIAL REASONS FOR CONVENIENCE
The small print
• Under the terms of working, doctors are required to consider home visits for MEDICAL REASONS ONLY
• If you think you qualify for a home visit please ring in the morning as early as possible.
• All visit requests will be medically assessed to check if appropriate

Home Visits, whilst convenient, offer a poor standard of care compared to a surgery visit because there may be lack of proper facilities/ poor access/lack of patient records and chaperones if an examination is required.

WHAT IS THE DEFINITION OF HOUSEBOUND

when you are unable to leave their home environment through a physical or psychological illness. A patient is not considered housebound if he or she is able to leave their house with minimal assistance or support. For example: unassisted/assisted visit to the Doctor, dentist, hairdresser, supermarket, social events or hospital outpatients. Some patients may not be housebound permanently but rather are housebound temporarily as a consequence of an episode of illness". Minimal assistance would be described as a person who can leave their own home and travel to a clinic appointment in a vehicle such as a personal car, taxi or public transport adapted for their use or not and with or without the use of a wheelchair either by themselves or with an es**rt. To avoid confusion any person who requires a specialist vehicle (Ambulance) or a two person es**rt would be regarded as housebound. To avoid further confusion a person will not be regarded as housebound because they do not personally have a companion to es**rt them to a clinical
appointment.

Following the death of Her Majesty Queen Elizabeth II and the activation of arrangements relating to her lying in state and subsequent funeral.

The government has confirmed that Monday 19th September will be a Bank Holiday.

NHS England and Integrated Care Boards (ICBs) have the responsibility to ensure patients can access urgent primary care services during this time and we would like to reassure our patients that ongoing access to NHS Urgent Primary Care Services will be available during this time.

The GP Practice will be CLOSED on Monday 19th September 2022.

Out-of-hours services will be in place during what would have been core hours to meet patient’s urgent primary medical care.

In planning for this day, primary care commissioners and providers have considered the closures amongst schools and childcare settings, impacting childcare arrangements. Supermarkets/large stores may also close or operate reduced opening hours impacting in-store pharmacy opening.
Please only call during this time if your problem is urgent (0115 929 2700) and please order any medication requests in plenty of time.

Any patients who have booked appointments during this time will be contacted to re-arrange a suitable date and time.

To all visitors attending the practice. This is what was collected outside our practice this morning. We provide bins both inside and in our foyer for face masks to be thown away after use. Leaving them littered around the practice makes YOUR practice look untidy. Please be respectful and use the bins provided. Thankyou

Notice from Infection Prevention Team 5th July 2022
For ALL patients & staff attending Aspley Medical Centre

COVID cases are increasing across Nottingham and Nottinghamshire, this impacts the system with increased admissions and increasing community COVID outbreaks.

For those practices that moved to a risk based approach with mask wearing you must now move back to universal face masks for both staff and patients to reduce transmission within the community.

All practice staff should wear a type 11R face mask and patients should wear a face mask/covering when attending the practice and if they have respiratory symptoms or confirmed infection must be provided with a type 11R face mask.

DID NOT ATTEND OR CANCEL APPOINTMENT NOTICE

We understand that very occasionally that there may be mix-up or communication error in providing patients appointments. However, booking appointments and not attending without informing the practice (DNAs) means that someone else who may desperately need an appointment is denied medical care. We do not feel that this is acceptable behaviour from patients. Not only is this costly to the NHS but this is also detrimental to the health of our other patients.
IN APRIL ALONE 37 hours OF OUR APPOINTMENTS WERE WASTED BY PATIENTS NOT ATTENDING A PRE-BOOKED APPOINTMENT.

2210 MINUTES WERE WASTED - THATS AN EXTRA 221 APPOINTMENTS WE COULD OF OFFERED TO OTHER PATIENTS.
THIS COSTS THE NHS £7,956!! OF WASTED TIME & MONEY!

As a practice we are always looking at ways to improve our services and this is one area we feel needs to be addressed to support the NHS and help us to offer extra appointments to our patients.

Therefore if you 'DO NOT ATTEND' your appointment or ring/text to cancel this will
go down in your record as a did not attend and if you do not cancel or don't attend upto 3 missed appointments in the year you will be removed from our practice list.
(Please DO NOT rely on a text reminder for your appointment as the system is not always reliable. It is your responsibility to ensure you attend your appointment on time or cancel if you cannot attend)

PLEASE HELP US TO HELP YOU BY CANCELLING YOUR APPOINTMENT IN ADVANCE IF YOU NO LONGER NEED IT. THANKYOU

Flu vaccine and coronavirus (COVID-19)
Flu vaccination is important because:

more people are likely to get flu this winter as fewer people will have built up natural immunity to it during the COVID-19 pandemic
if you get flu and COVID-19 at the same time, research shows you're more likely to be seriously ill
getting vaccinated against flu and COVID-19 will provide protection for you and those around you for both these serious illnesses
If you've had COVID-19, it's safe to have the flu vaccine. It will still be effective at helping to prevent flu.

Who can have the flu vaccine?
The flu vaccine is given free on the NHS to people who:

are 50 and over (including those who'll be 50 by 31 March 2022)
have certain health conditions
are pregnant
are in long-stay residential care
receive a carer's allowance, or are the main carer for an older or disabled person who may be at risk if you get sick
live with someone who is more likely to get infections (such as someone who has HIV, has had a transplant or is having certain treatments for cancer, lupus or rheumatoid arthritis)
frontline health or social care workers
