Today is the start of Somerset’s Advance Care Planning week 24!

We will be in Langport and Taunton libraries for a few hours today talking to anyone who would like to find out more!

Delighted to announce this this we will be putting on The Big Talk, answering all those important planning ahead questions! This is a FREE online event, open to everyone!

Somerset Advance Care Planning week is 17th - 22nd June this year!

Head to your local library to find out more and pick up your free Somerset Planning Ahead booklet* (*subject to availability).

Today is the final day of Advance Care Planning week 2023.

We will be at Taunton Library with Marie Curie 10-4 to answer any questions you have.

Please come in between 10-4pm to find out about the plans you can create to document your wishes and preferences about your future health and care.

On Thursday 29th we will have Somerset’s Treatment Escalation Plan lead talking through what is a STEP/ Do not resuscitate form and answering any questions you may have.

You can also join us for free crafts and other activities through the week- see the timetable attached!

Come along to find out about the different types of plans you can make. Marie Curie will be on site to help you create a free plan to document your wishes, preferences and goals.
You can also join in with free legacy craft activities 10:30- 12:00 in the library meeting room Monday - Thursday. On Friday 30th June between 10:30- 12:00 you can join in with a free creative writing session- writing your memoir.

Somerset End of Life Care Conference 2022 A countywide two day conference for all health and social care staff working with individuals with life limiting conditions

Some of the end of life care education team attended a webinar today ‘Enabling change through conversations about living and dying well’. Some important discussions occurred about supporting people and their families.

Great to hear Palliative Medicine Consultant Charlie Davis from Somerset FT sharing the work in Somerset such as:
▪️ Advance care planning (ACP) recording on SIDeR (Somerset integrated digital e-record)
▪️ Somerset Treatment escalation plans (STEP/TEP)
