A busy yet interesting week: A**l ca**l high-grade dysplasia treated with ESD. Photos depict a. the lesion prior to resection b. optimal use of saline immersion and cap during pulsed cut and submucosal dissection stages of ESD c. Cutting alongside Morgagni columns d. the abundance of vessels some of which corresponding to regional vascular plexus e. part of the dissection was perpendicular to the internal a**l sphincter fibers f. Final cut g. full mobilization of the lesion h. electrocautery at the dissection site/ESD defect. Saline, 1.5mm Dual J knife, short ST hood, 4mm Coagrasper, ESG 300 (PulseCut slow and forced coag) were used.

Πρώτη χρήση στην Ελλάδα του νέου συστήματος συρραφής Sutuart της Olympus για σύγκλειση εμμένοντος συριγγίου μεταξύ ορθού και ουροδόχου κύστης σε ασθενή με Ca ουροποιητικού και χειρ/κή εκτομή για Ca παχέος. Στις εικόνες φαίνονται: α. η είσοδος καθετήρα εντός του συριγγίου β. ο καυτηριασμός του στομίου και συριγγώδους πόρου από δύο διαφορετικές θέσεις με χρήση του additional working channel (AWC) της Ovesco γ. η είσοδος του ειδικού ράμματος V-LOC δια του πρωκτού με σύλληψη από το ράμμα (όχι από τη βελόνα) δ. η είσοδος της βελόνας και του ράμματος εντός του τοιχώματος δια του χάσματος του συριγγίου και η έξοδος της βελόνας ε. περιστροφή της βελόνας στ. πιο αποτελεσματική σύσφιξη του ράμματος/σύγκλειση χάρη στη χρήση 2 εργαλείων (Sutuart από κανάλι εργασίας ενδοσκοπίου και λαβίδα από AWC).
First use in Greece of the new Sutuart device to provide closure of a rectovesical fistula in an urinary tract Ca patient post-surgical resection for re**al Ca. Still frames show a. catheter inside the fistula tract b. cauterization of fistula or***ce and tract using endoscope's working channel and the additional working channel (AWC) by Ovesco c. introduction of the V-LOC suture inside the re**um while grasping the suture (not the needle) to allow free movement thus avoiding pinching/anchoring d. the needle pinching through the wall, fistula tact and finally rotating back into the lumen e. needle rotation f. effective tightening using two intruments, Sutuart (through the scope's working channel) and forceps (through the AWC).

The elusive nature of early gastric cancer. Even in HD systems such lesions can escape diagnosis as they may show a subtle "reddish" appearance under white light (kudos Dr Koukouratos from General Hospital of Larisa for detecting and getting reliable histology at a single bite). Narrow band imaging and indigo carmine chromoendoscopy are useful tools for detection and demarcation of these lesions. The lesion was removed using a classic/old school ESD approach. Marking, submucosal injection, circumferential incision (pulse cut slow effect 2 120W) and dissection (forced coag effect 2 45W) were performed using Dual j knife and a short ST hood. Exposed vessels were treated with Coagrasper 5mm (soft coag effect 4 70W). ESD defect was closed using 16mm hemoclips. ESD team A. Manolakis and Tr.Mpektsis (who also performed final cuts thus releasing the "suspended" lesion from the submucosa). Many thanx to our Anesthesiology team. Patient discharged two days later.
Μεγάλη εβδομάδα με ESD για πρώιμο γαστρικό Ca για το Department of Gastroenterology, University of Thessaly School of Medicine. Οι φωτο αναδεικνύουν τη δυσχέρεια στην αναγνώριση τέτοιων πρώιμων βλαβών και την αξία της χρήσης εικονικής αλλά και κλασικής με Indigo carmine χρωμοενδοσκόπησης για τον εντοπισμό και οριοθέτηση τέτοιων βλαβών. Οι λοιπές φωτό απεικονίζουν στάδια από την ESD. Χρήση short ST hood, oριοθέτηση/σήμανση και διατομή με Dual j knife, καυτηριασμός αγγείων με Coagrasper 5mm, σύγκλειση με clips 16mm. En bloc αφαίρεση-καθήλωση δείγματος με Insulin needles σε nylon-coated cardboard.

Υποτροπιάζουσα αιμορραγία από εκκολπώματα που αντιμετωπίστηκε με χρήση ελαστικού δακτυλίου περίδεσης. Στις φωτό απεικονίζεται το ανεστραμμένο εκκόλπωμα και το υπεύθυνο για την αιμορραγία αγγείο , μέσα από το διαφανή κύλινδρο που φέρει τους ελαστικούς δακτυλίους.

An 87-year-old male with a Zenker's diverticulum and a history of dysphagia and aspiration pneumonia underwent flexible endoscopic septum division (septotomy). A guidewire was introduced to the esophagus, followed by a nasogastric tube (keeping the esophageal entry identifiable and open). The use of the Stag Beetle knife (SB knife) for incision and dissection clearly facilitated and accelerated the procedure. A short ST hood and an ESG300 electrosurgical unit were also used. The procedure ended with placement of two lockado clips and tube removal under endoscopic supervision. Endoscopy team : A. Manolakis, K. Argyriou, Tr. Mpektsis. An interesting yet challenging case: Recurrent lower GI bleeding in a patient with cirrhosis. Upon colonoscopy a rather large lesion with a small rupture&clot was identified in the ascending colon. Initial management with placement of 2 hemostatic clips ceased bleeding for 48h. A CT scan was performed to clarify the nature of the lesion as there was a high suspicion for an ectopic, ascending colon varix. Upon recurrence, histoacryl+lipiodol (1:1) was slowly injected into the varix leading to its successful occlusion.
Photos illustrate the varix, glue injection, palpation of the varix post-injection, scope withdrawal with protruding catheter to avoid glue cohesion on and within the scope as well. X ray image illustrates glue-lipiodol mixture within the occluded variceal branches (bottom left) as well as cholelithiasis (upper left).

Two POEM cases in a row for us last week!! The first patient exhibited type II Achalasia, based on HRM yet extensive spastic activity on esophagram and endoscopy. Panpressurization intrabolus pressure pattern "masked" the spastic activity. That is why we consider HRM and esophagram complementary diagnostic tests. The patient underwent an extensive myotomy (15cm) that led to resolution of symptoms (dysphagia and retrosternal chest pain). During tunneling strong spastic contraction "decapitated" the tip of the short ST hood... TTJ knife was utilized for dissection and myotomy.
The second case was a patient with type I Achalasia, based on HRM, with profoundly increased IRP and LES resting pressure. Extensive fibrosis was also encountered during submucosal tunneling. Patience (not haste) and meticulous dissection led to a succesful result as shown in the last lax EGJ frame. Many thanx to our Anesthesiology team (featuring Dr Papaspyrou) led by Professor Arnaoutoglou and Professor Zacharoulis Head of Surgery. Congrats to Dr Chougias for excellent assistance during both POEMs. Additional frames depict tunneling, myotomy and clipping parts of POEM.
Δύο από του στόματος ενδοσκοπικές μυοτομές (POEM) για το τμήμα μας την προηγούμενη εβδομάδα! Ο πρώτος ασθενής εμφάνισε Αχαλασία τύπου ΙΙ μανομετρικά αλλά οισοφαγογραφικά και ενδοσκοπικά στοιχεία ισχυρού και εκτεταμένου σπασμού. Η στήλη παν-συμπίεσης κατά τη μανομετρία απέκρυψε το σπασμό. Για αυτό το λόγο θεωρούμε τη μανομετρία και το οισοφαγογράφημα συμπληρωματικές εξετάσεις. Εκτεταμένη μυοτομή 15cm οδήγησε σε εξάλειψη της δυσφαγίας και του οπισθοστερνικού άλγους που εμφάνιζε ο ασθενής.
Το δεύτερο περιστατικό εμφάνιζε μανομετρικά Αχαλασία τύπου Ι με υπέρμετρα αυξημένη IRP και πίεση ηρεμίας του κάτω οισοφαγικού σφιγκτήρα. Εκτεταμένη ίνωση διαπιστώθηκε κατά τη διάνοιξη της υποβλεννογόνιας σήραγγας. Υπομονετική (χωρίς βιασύνη) και προσεκτική διατομή οδήγησε σε επιτυχές, ανεπίπλεκτο αποτέλεσμα, όπως μαρτυρά και η χαλαρή γαστροισοφαγική συμβολή μετά την POEM. Πολλές ευχαριστείες στην Αναισθησιολογική μας ομάδα (με την Δρ Παπασπύρου) υπό την καθ. κα Αρναούτογλου και τον καθ. Χειρουργικής κο Ζαχαρούλη για την υποστήριξη. Πολλά συγχαρητήρια και στον Δρ Χούγια που εκτέλεσε άριστα χρέη πρώτου βοηθού και στις δύο POEM.

A challenging yet interesting POEM case. A young patient with type II Achalasia underwent Heller myotomy& Dor fundoplication followed by a second surgical myotomy a few months later. As symptoms persisted he was offered POEM as a "rescue" therapy. Captions from POEM procedure can be found below: a&b. initial incision and submucosal tunneling at 5 o clock (posterior wall) to avoid the surgically dissected plain c&d. extensive and thick fibrosis especially within the fundoplication wrap warranting careful dissection e&f completion of tunneling and subsequent selective myotomy of circular muscle fibers g. An endoscopic septotomy of a Zenker's diverticulum performed in our Department by our endoscopy team (Ass.Prof. Anastasios Manolakis, Consultants Dr Korina Pappa and Dr Kostantinos Argyriou) using a clutch cutter knife: a. Following nasogastric tube placement (can be seen beside the septum, transversing the esophageal opening) first incision is performed on the septum b. deeper incision to reveal the c. muscle fibers of the septum d.further dissection of muscle fibers e. placement of clips at the end of the procedure.
Πραγματοποίηση στο τμήμα μας ενδοσκοπικής θεραπείας εκκολπώματος Zenker από την ενδοσκοπική μας ομάδα με χρήση clutch cutter knife: α. Μετά την τοποθέτηση ρινογαστρικού καθετήρα (διακρίνεται παρά το διάφραγμα του Zenker στην είσοδο του οισοφάγου) πραγματοποιείται η πρώτη τομή β. εν τω βάθει επέκταση της τομής ώστε να αποκαλυφθούν γ. οι μυικές ίνες του διαφράγματος του Zenker δ. περαιτέρω διατομή μυικών ινών ε. τοποθέτηση clips στο τέλος της παρέμβασης.

An Endoscopic Stricturotomy/Stricturoplasty performed in our Endoscopy Unit on a patient with long-standing, recurrent anastomotic stricture despite repeated balloon dilatations, in an effort to avoid revision. Initially, a 9.2mm endoscope could transverse the stenosis whereas post-stricturotomy a diameter of 18mm could be achieved. Endoscopic images correspond to pretreatment condition, partial "trimming" and dissection of the anastomotic stricture using IT-nano knife, final result with clips to maintain result and remodelling of the anastomosis, follow-up after 3weeks. Endoscopic team comprised of Assist.Professor A. Manolakis and Dr F.Fytsilis. Extra credits to Professor D. Zacharoulis for surgical consultation and Multidisciplinary Team handling of the case.
Ενδοσκοπική διατομή&πλαστική στένωσης στο τμήμα μας σε ασθενή με ιστορικό εμμένουσας αναστομωτικής στένωσης παρά τις ενδοσκοπικές διαστολές προς αποφυγή χειρουργείου. Αρχικά ήταν η δυνατή μόνο η διέλευση λεπτού ενδοσκοπίου (9.2mm) από τη στένωση ενώ με την ενδοσκοπική διατομή/πλαστική επετεύχθη διάμετρος της τάξης των 18mm. Ενδοσκοπικές εικόνες πριν την παρέμβαση, κατά τη σταδιακή διατομή της στένωσης με μαχαιρίδιο IT-nano, τελικό αποτέλεσμα με clips για τη διατήρηση του αποτελέσματος και remodelling της αναστόμωσης και εικόνα κατά τον επανέλεγχο 3 εβδομάδες αργότερα. Ενδοσκοπική ομάδα Επίκ.Καθ. Α. Μανωλάκης και Δρ Φ/Φυτσιλής. Ειδική μνεία στον Καθ. Δ. Ζαχαρούλη για την από κοινού διαχείριση του περιστατικού στα πλαίσια διεπιστημονικής ομάδας.

Another Per-oral endoscopic myotomy (POEM) was completed in a patient with Achalasia and past Pneumatic Dilatation by our POEM team: Ass.Prof. Α. Manolakis, Dr Αik. Pappa and Dr Ap. Papaefhtymiou. Many thanks to Professor A. Kapsoritakis&Dr Ev.Makrygiannis for patient-related guidance, our Anesthesiology team led by Professor Arnaoutoglou and our General Surgery team led by Professor Zacharoulis for their support. POEM snapshots can be seen below: a. Tunneling using TTj knife and short ST hood-spray coagulation current generated from an ESG-400 unit creates a neat spark b. Left gastric artery branch also known as Tanaka vessel c. Longitudinal muscle fibers have been exposed while the tip of the knife hooks and dissects circular muscle fibers d. A lax EGJ is evident post-myotomy e. Closure of incision with 20mm and 16mm clips.
Μία ακόμη επέμβαση POEM (από του στόματος ενδοσκοπική μυοτομή) ολοκληρώθηκε στο ΠΠΓΝ Λάρισας για τη θεραπευτική αντιμετώπιση Αχαλασίας σε ασθενή με προηγούμενη Αεροδιαστολή από την "Ομάδα POEM": Eπίκ.καθ. Α. Μανωλάκης, Δρ Αικ. Παππά και Δρ Απ. Παπαευθυμίου. Πολλές ευχαριστείες στον καθ. Α. Καψωριτάκη&στον Δρ Ε.Μακρυγιάννη για την σχετιζόμενη με τον ασθενή καθοδήγηση, στην Αναισθησιολογική ομάδα υπό την ηγεσία της καθ. Ε. Αρναούτογλου και τη Χειρουργική ομάδα υπό τον καθ. Δ. Κλάδος της αριστερής γαστρικής αρτηρίας επί τα αριστερά, γνωστός και ως αγγείο Tanaka γ. Οι επιμήκεις μυικές ίνες έχουν αποκαλυφθεί ενώ το μαχαιρίδιο "αγκιστρώνει" τις κυκλοτερείς μυικές ίνες για να ακολουθήσει η διατομή τους δ. Χαλαρή γαστρο-οισοφαγική συμβολή μετά από τη μυοτομή ε. Σύγκλειση της τομής εισόδου με clips των 20mm(Lockado) και 16mm (SureClip).

The Department of Gastroenterology, University of Thessaly, School of Medicine has succesfully participated in two major endoscopic events: European Society of Gastrointestinal Endoscopy (ESGE) days 2022, in Prague and World Endoscopy Organisation (WEO) World Endo 2022 in Kyoto. Endoscopic videos and e-presentations were succesfully presented/displayed during both congresses. Congratulations to all authors and presenters. Keep it up!
Η Πανεπιστημιακή Γαστρεντερολογική Κλινική της Ιατρικής Σχολής του Πανεπιστημίου Θεσσαλίας συμμετείχε σε δύο μείζονα ενδοσκοπικά συνέδρια, ένα Ευρωπαϊκό στην Πράγα και ένα Παγκόσμιο στο Κιότο. Ενδοσκοπικά βίντεο και ηλεκτρονικές παρουσιάσεις προβλήθηκαν και παρουσιάστηκαν με επιτυχία κατά τη διάρκεια και των δύο συνεδρίων. Συγχαρητήρια σε όλους τους συγγραφείς και "παρουσιαστές"-ομιλητές!

For a second consecutive year, United European Gastroenterology organisation (UEG) grants us the honorary title of Host Centre for Clinical Visiting Fellows. We eagerly await for International Fellows to join our training program.

On February 20th, during the 2022 International Speaker Sessions, the Department of Gastroenterology, University of Thessaly School of Medicine will host a prestigious interventional endoscopist from USA, Dr Ashish Sharma. Dr Sharma will offer insight on closure techniques in the GI tract (perforations, fistula, EMR/ESD defects etc) and state-of-the-art EUS interventions (perhaps during a second session). Dr. Sharma earned his medical degree from Osmania Medical College in Hyderabad, India. He completed an internal medicine residency at Saint Vincent Hospital in Worcester, Massachusetts. He completed his general gastroenterology fellowship at University of Massachusetts, followed by an advanced endoscopy fellowship at the University of California, Irvine. He completed an advanced endoscopyµsurgical fellowship in Tokyo with professor Haru Inoue and served as a visiting fellow at the prestigious National Cancer Center in Tokyo. He has also served as a clinical instructor at Boston University School of Medicine and the University of California, Irvine. Dr. Sharma served on the faculty of Harvard Medical School, and more recently, established a center of excellence in advanced endoscopy at Kaiser Permanente Medical Group in Sacramento, California. He is currently head of therapeutic endoscopy at the Lutheral Health Network in Fort Wayne, Indiana. Apart from basic endoscopic procedures Dr Sharma engages in ERCP, EUS, Enteroscopy(DBE&spiral), RFA, cryoablation, Confocal Laser Endomicroscopy, POEM/POET, ESD.
