Save the Limb
People suffering from peripheral vascular disease and often poorly treated and it results in many people losing their limbs. Many a times toes and feet are cut off for nonhealing ulcers. Here I a shwoing a case of nonhealing ulcer of right great toe. The case was referred by surgeons and on evaluation was found to have occlusion of all the blood vessels (anterior tibial artery, posterior tibial artery and peroneal artery) supplying blood to the foot. Such cases are technically very challenging to perform. In this case it was not possible to cross the occlusion from above, so we did a puncture of the Dorsalis pedis artery and did a retrograde wiring. Angioplasty of the anterior tibial artery was done and good flow was established.

HYBRID REPAIR OF AORTA
A 50 years old female presented with complaints of severe pain in the upper back and chest. On evaluation was found to have dissection of the aorta. Dissection of aorta is a serious problem. Aorta is the main artery arising from the heart and carrying blood to the body. In dissection there is a tear inside the aorta. With the advancement of therapy, now we are able to manage many cases of aortic dissection by stenting the aorta. In this case there was a large dissection flap which was extending to the left subclavian artery. So in this case our cardiac surgeons first did carotid to carotid and carotid to left subclavian bypass. Then the Cardiology team did the endovascular repair. This is a one of its kind hybrid repair of aorta in Odisha. KIMS has pioneered in many aspects of Cardiology including first to do Transcatheter Aortic valve replacement (TAVR), EPS RFA, Endovascular repair of Aorta and many more.

Carotid artery stenting.
Carotid arteries supply blood to the brain. Some types of brain strokes are caused by blockages in these arteries. One treatment modality for the blockages in these arteries is to put a stent to normalize blood flow to the brain also to prevent the migration (embolization) of atherosclerotic materials into the brain. The pictures here show the presence of a very ulcerated plaque in a carotid artery. This patient suffered a minor brain stroke and recovered. We deployed a stent to the carotid artery with the aim preventing future cerebrovascular accident. The procedure was done at KIMS

Our understanding of diet and nutrients is evolving over time. Though the traditional approach is to limit fat intake, new data shows fats to be protective. Full fat dairy, high fat nuts (Peanuts, cashews, almonds) are protective.
A review article in JACC outlines the following commentary
"The recommendation to limit dietary saturated fatty acid (SFA) intake has persisted despite mounting evidence to the contrary. Most recent meta-analyses of randomized trials and observational studies found no beneficial effects of reducing SFA intake on cardiovascular disease (CVD) and total mortality, and instead found protective effects against stroke. Although SFAs increase low-density lipoprotein (LDL) cholesterol, in most individuals, this is not due to increasing levels of small, dense LDL particles, but rather larger LDL particles, which are much less strongly related to CVD risk. It is also apparent that the health effects of foods cannot be predicted by their content in any nutrient group without considering the overall macronutrient distribution. Whole-fat dairy, unprocessed meat, and dark chocolate are SFA-rich foods with a complex matrix that are not associated with increased risk of CVD. The totality of available evidence does not support further limiting the intake of such foods."
