Toothbytes: Management of an Avulsed Lower permanent central incisor with open apex.
*Instructions were given to the individual by phone to store the tooth in a container with saline solution.
*Tooth was cleaned with a gentle stream of saline
*Pre & Post Radiographic examination was done to check for intact bone/ fracture & Reimplantation
*After reimplantation, tooth was stabilised with the adjacent non-mobile tooth with passive, flexible wire and composite
*Analgesics, Antibiotics, a Tetanus toxoid shot & Antiseptic mouth wash prescribed for 2 weeks
*Review every 2 weeks for pulp necrosis/ root resorption, if necessary to initiate apexification or RCT.

Endo-Perio Lesion Tx & Management:
*No Ca(oh)2
*No Multiple-visits
*No Periapical surgery/ Curettage/ Bonegrafts
*Just Single-visit RCT with proper C,S, starting from 10 size to 3 times enlargement of all canals from the 1st file that reaches the apical constriction supported Apex Locator, copious NaOCl irrigation & BC sealer with post-core followed by crown in the 2nd visit.
*Follow-up after 1yr

Retreatment of a Long, Narrow, Silver-Point obturated 46 with 4 canals:
A 45 yrs/Male reported due to pain & infection in his mandibular right first molar treated 10 yrs before
-IOPA radiograph with Mesial shift showed SILVER POINT OBTURATION in ML canal filled short of WL & 2 incompletely filled canals in Mesio-buccal, Disto-buccal & Disto-lingual canal
-The existing crown was removed, The silver point & all the GP were removed under microscopic magnification & all the canals were located and negotiated to patency. They were long, constricted and very difficult to negotiate
-The GP was hardened & extremely difficult to remove but was done with GP Solvent, H-files & Neoendo Rotary files with 3% NaOCl irrigation and obturated with 4% taper GP of 25 size coated with Angelus BC Sealer
-At the 2nd visit, after two weeks, the pain has subsided and the the tooth was crowned back.

A Life changing moment for a pt who has Severe Crowding & Malocclusion contraindicated for Invisible Clear Aligners, has undergone orthodontic treatment after all-4-extractions with Ceramic braces.

VDIC Dental Blog:
“Not all patients are candidates for extensive 2nd implant surgery when the 1st one had failed. This is one such pt who wanted a FIXED/SEMI-FIXED OPTION due to her age, medical condition & sufferings caused post implant surgery”.

Although there are options like Telescopic Dentures, severe proclination of the upper anterior implant abutments prevented its use in this case. Therefore a COMBINED PROSTHESIS with Extracoronal Precision attachment was made fulfilling the pts needs.
Finally we were able to see the pts smile for the FIRST TIME in 3 weeks after the insertion of the prosthesis😊

Finally, even though I prefer Implant supported crowns for such cases, it’s quite rewarding for the patient & personally that u were able to save the teeth at the end😊
