A 70-year-old male presented with difficulty in swallowing. He was evaluated and diagnosed with epiphanic esophageal diverticulum with narrowing at the lower end of the esophagus. CECT of the chest and abdomen confirmed endoscopic findings. After evaluation, he was taken up for surgery. Laparoscopic excision of the diverticulum cardiomyotomy and anterior fundal wrap under endoscopic guidance were done. The oral dye study did not reveal any leak from the staple line. He was started on an oral diet and was discharged on day 2 after surgery.
