Complex HPB series

A 57-year-old gentleman with obstructive jaundice due to a hilar stricture. Radiology and Spyglass Cholangioscopic biopsies did not yield a definitive diagnosis. After previously being stented elsewhere, we performed an extrahepatic bile duct excision and sent for Frozen biopsy, results revealed adenocarcinoma, leading us to proceed with Right Hepatectomy, Caudate Lobectomy, Extra biliary Tract Excision with Lymphadenopathy, and Left Roux En Y Hepatico-Jejunostomy.
The procedure went smoothly, Grateful for the opportunity to overcome diagnostic dilemmas!
