*What is PCOS?*

1. Lifestyle modifications: Adopting a healthy lifestyle with regular exercise and a balanced diet can help improve insulin sensitivity, regulate menstrual cycles, and promote weight loss.
2. Medications: Birth control pills, hormone therapy, and insulin-sensitizing medications may be prescribed to regulate menstrual cycles, reduce androgen levels, and improve fertility.
3. Fertility treatments: Women trying to conceive may benefit from medications to induce ovulation or assisted reproductive technologies such as in vitro fertilization (IVF).
4. Management of symptoms: Treatments for acne, excessive hair growth, and other symptoms of PCOS are available and may include topical treatments, oral medications, or procedures like laser hair removal.

First trimester screening test is a prenatal test that can help assess the risk of certain chromosomal abnormalities in the developing baby, such as Down syndrome, trisomy 13, and trisomy 18. It consists of a blood test and an ultrasound scan, usually done between weeks 10 and 14 of pregnancy. In this blog, I will explain what the test involves, what the results mean, and what are the benefits and limitations of the test.

The blood test measures the levels of two proteins in the mother’s blood: pregnancy-associated plasma protein-A (PAPP-A) and human chorionic gonadotropin (hCG). These proteins are produced by the placenta and can indicate how well the pregnancy is progressing. Low levels of PAPP-A and high levels of hCG may be associated with an increased risk of chromosomal abnormalities.

The ultrasound scan, also known as nuchal translucency screening, measures the thickness of the fluid-filled space at the back of the baby’s neck. This space, called the nuchal fold, tends to be larger in babies with chromosomal abnormalities. The scan also checks the baby’s nasal bone, which may be absent or underdeveloped in some cases of Down syndrome.

The results of the blood test and the ultrasound scan are combined with the mother’s age and other factors to calculate the probability of the baby having a chromosomal abnormality. The result is expressed as a ratio, such as 1 in 100 or 1 in 1000. A lower ratio means a higher risk, and a higher ratio means a lower risk. For example, a result of 1 in 100 means that there is a 1% chance of the baby having a chromosomal abnormality, and a result of 1 in 1000 means that there is a 0.1% chance.

The first trimester screening test is not a diagnostic test, which means that it cannot confirm or rule out the presence of a chromosomal abnormality. It only provides an estimate of the risk, based on statistical averages. Therefore, a normal result does not guarantee that the baby is healthy, and an abnormal result does not mean that the baby has a problem. Most babies with an abnormal result are born healthy, and some babies with a normal result may have a chromosomal abnormality.

The main benefit of the first trimester screening test is that it can provide early information about the baby’s risk of chromosomal abnormalities, which can help the parents make informed decisions about further testing and care. The test can also detect some other problems, such as major structural defects, fetal growth restriction, or miscarriage risk. The test is safe and noninvasive, and does not pose any harm to the mother or the baby.

The main limitation of the first trimester screening test is that it is not 100% accurate, and it may miss some cases of chromosomal abnormalities or give false positive results. The test also does not detect all types of chromosomal abnormalities, such as microdeletions or mosaicism. The test may not be suitable for some women, such as those with multiple pregnancies, obesity, or previous fertility treatments. The test may also cause anxiety or stress for some parents, especially if the result is abnormal.

The first trimester screening test is an optional test that is offered to all pregnant women. The decision to have the test or not is entirely up to the parents, and they should discuss the pros and cons of the test with their doctor and genetic counselor. The test is not a substitute for the second trimester screening test or the 20-week anatomy scan, which are also important for checking the baby’s health and development.
