Many areas in Davao City is under water again and many people are at risk to develop Leptospirosis which sometimes can be fatal in 10% of people who develop severe disease, including kidney failure and/or liver damage, meningitis, difficulty breathing, and bleeding. Case fatality rate is 5 to 15% in cases with severe clinical illness.

Here are the recommended post-exposure prophylaxis for leptospirosis from the Philippine Society of Microbiology and Infectious Disease (PSMID):

LOW-RISK EXPOSURE is defined as those individuals with a single history of wading in flood or contaminated water without wounds, cuts or open lesions of the skin.

Doxycycline 200 mg single dose within 24 to 72 hours from exposure [Grade B]

MODERATE-RISK EXPOSURE is defined as those individuals with a single history of wading in flood or contaminated water and the presence of wounds, cuts, or open lesions of the skin, OR accidental ingestion of contaminated water.

Doxycycline 200 mg once daily for 3-5 days to be started immediately within 24 to 72 hours from exposure [Grade C]

HIGH-RISK EXPOSURE is defined as those individuals with continuous exposure (those having more than a single exposure or several days such as those residing in flooded areas, rescuers and relief workers) of wading in flood or contaminated water with or without wounds, cuts or open lesions of the skin. Swimming in flooded waters especially in urban areas infested with domestic/sewer rats and ingestion of contaminated water are also considered high risk exposures.

Doxycycline 200 mg once weekly until the end of exposure [Grade B]

What Is Arthritis

Characteristics associated with hospitalisation for COVID-19 in people with rheumatic disease: data from the COVID-19 Global Rheumatology Alliance physician-reported registry 03/06/2020

OBJECTIVES
COVID-19 outcomes in people with rheumatic diseases remain poorly understood. The aim was to examine demographic and clinical factors associated with COVID-19 hospitalisation status in people with rheumatic disease.

METHODS
Case series of individuals with rheumatic disease and COVID-19 from the COVID-19 Global Rheumatology Alliance registry: 24 March 2020 to 20 April 2020. Multivariable logistic regression was used to estimate ORs and 95% CIs of hospitalisation. Age, s*x, smoking status, rheumatic disease diagnosis, comorbidities and rheumatic disease medications taken immediately prior to infection were analysed.

RESULTS
A total of 600 cases from 40 countries were included. Nearly half of the cases were hospitalised (277, 46%) and 55 (9%) died. In multivariable-adjusted models, prednisone dose ≥10 mg/day was associated with higher odds of hospitalisation (OR 2.05, 95% CI 1.06 to 3.96). Use of conventional disease-modifying antirheumatic drug (DMARD) alone or in combination with biologics/Janus Kinase inhibitors was not associated with hospitalisation (OR 1.23, 95% CI 0.70 to 2.17 and OR 0.74, 95% CI 0.37 to 1.46, respectively). Non-steroidal anti-inflammatory drug (NSAID) use was not associated with hospitalisation status (OR 0.64, 95% CI 0.39 to 1.06). Tumour necrosis factor inhibitor (anti-TNF) use was associated with a reduced odds of hospitalisation (OR 0.40, 95% CI 0.19 to 0.81), while no association with antimalarial use (OR 0.94, 95% CI 0.57 to 1.57) was observed.

CONCLUSIONS
We found that glucocorticoid exposure of ≥10 mg/day is associated with a higher odds of hospitalisation and anti-TNF with a decreased odds of hospitalisation in patients with rheumatic disease. Neither exposure to DMARDs nor NSAIDs were associated with increased odds of hospitalisation.

Covid-19 changed the way we look at viruses. We have to relearn what we learned in medical school. Everyday we learn something new of the disease. There is so much that we don’t know about this virus in the beginning and every now and then we still surprised at how the virus is unfolding. It totally changed the way we live our lives.

Treating Rheumatic Diseases in a COVID-19 Epicenter 28/05/2020

In these uncertain times, patients with rheumatic diseases represent a vulnerable population whose health and wellness are particularly threatened, not only by the risk of COVID-19, but also by reduced access to usual medical care (in-person clinic visits), potential treatment interruptions (eg, planned infusion therapies), and the ongoing shortage of hydroxychloroquine, to name a few.

Maintaining an open line of communication with our patients (by phone or telemedicine) has become more essential than ever. It is through these communications that we best understand our patients' concerns and provide support and personalized treatment decisions.

The COVID-19 pandemic has presented us all with unprecedented challenges. As rheumatologists, it is our duty to lead our patients through this uncharted territory with close communication, information, advocacy, and personalized treatment decisions. Each of these is central to the management of rheumatology patients during the COVID-19 pandemic.
