Do you need an approved absence due to illness because you missed class? 😷 If yes, you need to obtain a validated medical certificate via the Health Services Office.

The main clinic is located on the ground floor of Bro. Connon Hall with the following hours:
🏥 8:00 am – 11:00 am on Mondays to Fridays
🏥 9:00 am – 11:00 am on Saturdays
🏥 1:00 pm - 4:00 pm on Mondays to Fridays

The satellite clinic is located on the ground floor of the Enrique Razon Sports Complex with the following hours:
🏥 8:00 am – 11:00 am on Mondays, Wednesdays, and Fridays only

The detailed procedures are as follows:

Ensure that the medical certificate is legibly written and includes the following:
📝 Official letterhead from the hospital, clinic, or medical facility
👨‍⚕️ Printed full name and signature of the attending physician or dentist
💉 License number of the physician or dentist
📆 Clinic schedule and active contact details of the attending physician or dentist

Bring the following documents:
📜 A hard copy of the original Medical Certificate or Certificate of Confinement
📋 Supporting documents (e.g., prescription, diagnostic test results)
📋 Photocopy of the medical certificate and supporting documents

Process your request with the HSO within 7 working days from the day you return to your classes. Incomplete or non-compliant requests will not be entertained or processed. ⏳

Once the Medical Certificate is validated, the HSO will issue an endorsement (yellow) slip to the requesting student. Note that the original medical certificate will be collected by HSO for filing in the student’s medical record. 📃

The student should submit the HSO yellow slip along with the attached photocopy of the validated medical certificate to the office of the Associate Dean via this form: https://forms.gle/zMdkMvKUWBrTnNSh9 or scan the QR code provided below. 📤

Please note that for reasons other than illnesses or sickness, you should communicate directly with your professor. 📢
