Please join me in welcoming Lauren Magana! She will servicing Dekalb County area and
Northern Gwinnett.

Lauren Magana is a Licensed Master Social Worker with over seven years of care management experience. She was inspired to become a social worker after watching her mother find her passion in hospice work. She began working with the LGBQTIA+ population in Atlanta serving unhoused youth in their journey towards identity building, stability, and life skills. She then pivoted into a combination of specialized healthcare and geriatrics while working in a skilled nursing facility and long-term care. From there, she worked alongside a team of attorneys in Elder Care Law, helping to guide families through aging, estate planning, and Medicaid law. Afterwards, she worked as both a hospice social worker and an Intensive Outpatient Therapist helping individuals, groups, and families via telehealth. Furthering her specialized knowledge, she explored chronic illness social work in renal/dialysis case management. With her wide range of experience, she now serves as a geriatric care manager.

Our online support groups provide a convenient and accessible way for care providers to connect with others from the comfort of their homes.
