During our Perfect Visit™, our staff come prepared to provide our patients and their families with excellent care. We encourage our Registered Nurses and Certified Nursing Assistants to be take time to walk our patients through what we are doing and make them feel like the hero. They provide our patients with information regarding their condition and what they can expect. From there, our staff with compile a list of items necessary to care for the patient after we leave. If they need refills or new medication to treat a new condition, we will take care of it. We leave our patients and their families feeling like their hospice needs are being taken care of so they don't need to do it themselves.

Caity Holder leads Sonder’s assisted living and nursing facility focus as our Community Wellness Nurse. She is a veteran clinical leader, with experience as a former assisted living wellness director and nursing facility ADON. In addition, Caity is a certified wound care nurse. She works closely with the Sonder communities we serve as an expert clinical resource with the goal of ensuring Sonder delivers a world-class experience for your families and team members.
