Heritage Park Rehabilitation and Skilled Nursing Center
We provide short and long-term skilled nursing care that helps older adults achieve true wellness. We are a non-profit, and thatโs an important difference.
Our focus is on strengthening the families and communities we care for. Post-operative care, Wound Care, IV Therapy, and an in-house rehabilitation team are some services that make our facility different. Unlike others who contract their therapists, we have an in-house therapy team. This means our goal is to have patients and residents see the same occupational, speech, or physical therapists daily, which builds a strong line of trust and rapport.

With strong community support, including visionary leadership from area hospitals and organizations, Hospice Austin expanded its services, becoming Medicare-certified in 1986 and extending its reach to surrounding counties! Amazing, right?
In 1997, Hospice Austin relocated to its present location and obtained . Previously serving as an in-patient AIDS facility, it was repurposed into the area's inaugural in-patient hospice facility.

As November comes to a close, we want to take a moment to honor Native American Heritage Month and recognize the history, ancestry, and traditions. First known as 'American Indian Day' and declared in 1916 by the State of New York, it wasn't resolved until 1990 by President George H. W. Bush and has since been an annual declaration.
โThis month is an opportunity to take a moment and reflect on the land that we currently have been on for so many years and the life that we built here as a country,โ says Paywand Baghal, a medical student at Texas Christian University.
