Another successful HoLEP launch! Grateful for the support and helpful crew at West Valley Hospital in Caldwell, Idaho. This is the 3rd location opened in 10 months, furthuring access patients have to state of the art treatment options.

Happy to report setting a world record for smallest instrumentation to perform a HoLEP this week. In the past the morcellation aspect of the procedure has required dilating the urethra to least 24Fr, this was avoided by using the 18Fr Myosure Lite gynecologic tissue shaver thereby minimizing trauma and dilation of the urethra.

Where did the term “failed medical management” originate for BPH management?
The first references in the literature trace back to ~1995 as criteria to participate in some surgical clinical trials for BPH. This was just a few years after the introduction of the alpha blocker Prazosin in 1992, and 2 years before the introduction of Tamsulosin in 1997. The surgical options at that time were predominantly monopolar TURP and open simple prostatectomy, both of which are rarely performed today due to their legendary risks of bleeding, prolonged hospitalization, and life-threatening hyponatremia. Bipolar TURP and the superior HoLEP were both just becoming available in the late 90s and weren’t surgical options when the term “failed medical management” started working its way into the urology vernacular fueled by the well-funded pharmaceutical industry.
Given the numerous advances in minimally invasive BPH treatment options today, now roughly 30 years since “failed medical management” got its start, we’re well past due to critically re-evaluate our outdated terminology. With the abundant harmful medication side effects now recognized, perhaps the justification for starting medications should be “ineligible for procedural intervention” so we can efficiently save more bladder from pharmaceutical neglect, and maintain our patient’s independence from quality of life impairing BPH symptoms.
