HIRING LPN & RN for Home Health position...

Position Overview: Proclaim Home care is seeking a part-time Licensed Practical Nurse (LPN) & Registered Nurse (RN) to join our team. The LPN/RN will deliver dedicated care to home health clients in Bartow, Floyd, Gordon & Whitfield areas. The ideal candidate must be punctual, reliable, and capable of managing care plans effectively.

Responsibilities:
*Provide direct care to home health clients
*Verify plan of care with the Director of Nursing prior to each visit.
*Collaborate with office staff to ensure patient needs are being met.
*Teach patients and caregivers self-care techniques
*Check vital signs
*Review all medications each visit
*Complete documentation timely and accurately
*Supervisory visits of aide care
*Complete admissions of services (RN only)

Qualifications:
*Current nursing license in the state of Georgia
*Current CPR certification
*Valid driver's license, reliable transportation and liability insurance.

Employment Type:
*Part-Time (16-20 hrs per week)
*Flexible schedule (can make your own weekly schedule)

PROCLAIM HOME CARE is currently accepting new clients for home health services.

Personal Support Services include: personal care aide to help with activities of daily living (light housekeeping, assistance with drs appts and errands, meal prep, hygiene care, and much more).

Skilled Nursing (RN/LPN) include: medication setup and management.

CCSP & SOURCE Medicaid waiver programs
Structured Family Care & Family Hire programs
Private Pay
Long-Term health policy coverage
Workers Comp
