Metabolic Syndrome: an often undiscussed and dangerous diagnosis.
Metabolic Syndrome is present if 3 out of 5 of the following abnormal findings exist:
1) Waist circumference greater than 35 inches in women, 40 inches in men
2) Elevated triglycerides above 150
3) A low HDL cholesterol level (the โ€œgoodโ€ cholesterol)
4) Elevated blood pressure, even borderline high blood pressure
5) A fasting glucose level greater than 100.
The reason we care so much about Metabolic Syndrome is because it increases a personโ€™s risk of developing diabetes, heart disease, stroke or all 3.

When is the best time to lose weight?

The importance of strength training during weight loss:
When people lose weight, they lose about 70% of their weight from body fat and the other 30% from lean body mass which includes weight from muscle, bone and body water.
Resistance or strength training while losing weight will decrease the amount of muscle lost and will increase the amount of fat lost.
Why else should people do strength training?
1) Strength training decreases body fat.
2) Strength training increases lean muscle which is makes weight loss more lasting and sustainable.
3) Strength training prevents the decrease in metabolism that is usually seen with weight loss.
4) Increased muscle mass/the avoidance of loss of lean body mass and muscle will decrease the tendency to overeat.
5) Strength training also lowers blood pressure, cholesterol and glucose levels.
6) Strength training decreases bone loss which can occur with weight loss and reduces the risk of developing osteoporosis.

What is Binge Eating Disorder?
Binge Eating Disorder is the most common eating disorder, affecting 3% of the population. It is characterized by consuming large quantities of food in a short period of time and feeling like you cannot stop eating.
Episodes of Binge Eating occur at least once weekly for several months.
People experience feelings of distress and shame after the episode of Binge Eating.
Complications of Binge Eating Disorder include Obesity, Type 2 Diabetes, Gallstones, Fatty Liver Disease, Kidney Disease and Arthritis.

Weight loss success!

We are seeing higher levels of weight loss at our Obesity Medicine Practice than we have for the last 5 yearsโ€ฆ. the secret: using one of the newer anti-obesity medications like Wegovy , Semaglutide, or Mounjaro along with a Low-Calorie Medical Meal Replacement Program.
The VLCD (very low calorie diet) or LCD (low calorie diet) achieved using a medical meal replacement program is a lot easier to sustain with minimal hunger when using a medication like Wegovy, generic Semaglutide or Mounjaro for appetite suppression.

The truth about exercise and weight loss:
Exercise is an important part of the weight loss picture, but oftentimes exercise alone does not allow for good weight loss, especially as we get older.
How much exercise is needed?
1) For general good health we need to do 150 minutes per week of moderate exercise or 75 minutes per week of vigorous exercise,
2) For the prevention of weight regain we need to do 300 minutes per week of moderate exercise or 150 minutes per week of vigorous exercise
3) For weight loss we need to do 420 minutes per week of moderate exerciseโ€ฆโ€ฆthis equals 60 minutes daily!
Exercise is critical for good overall health, but it does take a very high amount of exercise daily to promote weight loss when exercise is used alone; therefore, exercise is only a part of the picture for long-term weight loss.
Successful weight loss requires not only regular exercise, but also good nutrition, decreased portions sizes, and behavioral modification to create better eating habits that are realistic and sustainable. The FDA-approved anti-obesity medication are very helpful at making eating less food easier.

Semaglutide for weight loss:

The GLP-1 agonists Mounjaro, Ozempic and Wegovy comprise a new and very powerful group of medications for weight loss. Sadly, they are extremely expensive and are often on backorder at the pharmacies.

Menopause is associated with weight gain: specifically weight gain around the belly. This weight gain is sudden and stubborn, and although hormone replacement therapy can help a little with the weight gain in the abdomen, it does not cause overall weight loss.
Causes of weight gain during menopause:
-depleted estrogen
-anti-depressant medications often used for menopausal mood changes
-sleep disturbance
- a true decrease in the metabolism
-decreased physical activity
The menopausal weight gain in the belly does lead to an increased risk of heart disease so must be taken seriously!

The Treatment:
-a healthy diet with high protein
-exercise for more than 150 minutes weekly
-resistance/strength training at least twice weekly
-behavioral support
-management of any anxiety or depression
-work on sleep and stress.

Ways to boost your metabolism:
There are some determinants of metabolism that we canโ€™t change like age, gender and genetics, but there are some proactive things we can do to naturally boost metabolism:
1) Increase the nutrients in your diet by adding more greens, berries, nuts, seeds and high-quality protein.
2) Do more strength training to build up muscle: more muscle mass helps to burn fat.
3) Improve your sleep: work on good sleep hygiene habits with a goal of getting at least 7 hours of sleep each night.
4) Decrease stress: lowering your stress level will help to reduce excessive levels of cortisol (our main stress hormone) which will increase metabolism. Some simple ways to decrease stress are adding more exercise and using a meditation app.
5) Discuss you medication list with your doctor: many commonly used medications can cause an increase in hunger and a slight decrease in metabolism.

What is a VLCD and is it right for me?

VLCD stands for a Very Low Calorie Diet, which is usually a diet low enough in calories while still being nutritionally complete for rapid and safe weight loss. A VLCD diet usually consists of high protein shakes, puddings, bars and soups in a program made specifically for each patient. These restrictive diets are made successful and safe by frequent physician monitoring and support, as well as with the use of FDA-approved anti-obesity medications that take the edge off of both hunger and cravings.
Anyone with a BMI (Body Mass Index) over 30 or over 27 with weight related medical issues such as high blood pressure, sleep apnea, type 2 diabetes, arthritis or heart disease can qualify for this type of nutrition program.

What are the healthiest fruits and vegetables?

Although all fruits and veggies have health benefits, a few stand out as superior!
1) Broccoli: contains phytonutrients which help reduce the risk of developing heart disease, diabetes and some cancers.
2) Apples: contain antioxidants which have been shown in studies to improve the lifespan and reduce chronic disease.
3) Kate: contains loads of nutrients and is an especially good source of vitamins C and K.
4) Blueberries: an excellent source of fiber, antioxidants and phytonutrients; these berries may protect against memory loss and cognitive decline.
5) Avocados: can increase the level of HDL, the โ€œgoodโ€ cholesterol
6) Sweet potatoes: a healthy carbohydrates that contains loads of fiber, vitamin A, C and B6.

You are what you eat.

All calories are not created equally. A diet that is high in nutritious foods vs. a diet that contains the same number of calories with LESS nutritious foods will allow patients to have more weight loss and to feel more FULL!

When calories are not from healthy foods, the appetite will often increase to make a person eat more with the hope that the next calories will be nutritious ones. This means that when people eat unhealthy foods, they find it hard to feel full and satisfied and generally eat more.

Changing your diet to one that is made up of good sources of protein, fruits and vegetables, and whole grains will lead to a better sense of fullness naturally. This allows us to reduce our calories over time with less hunger.

Eating dos and donโ€™ts to support a healthy metabolism:
1) Get adequate protein at every meal (at least 15-20 grams)
2) Eat whole grains instead of white bread, white pasta, and white potatoes
3) Add fiber to your diet
4) Eat fruits and vegetables every day
5) Drink at least 60 ounces of water daily
6) Avoid processed foods
7) Avoid sugar
8) Avoid fried foods
9) Keep alcohol intake to a minimum (these are just empty calories)

Other things that you can do to enhance metabolism:

1. Get at least 8 hours of sleep each night
2. Do strength training 2-3 days weekly to build muscle
3. Eat a diet high in nutritious foods
4. Eat the correct amount of protein daily for your weight (a doctor can calculate this for you)
5. Start your day with a high protein breakfast

Weight loss success: The more supportive tools added on to a healthy diet alone increase the amount of weight loss per year:

1. Diet and exercise: 3-10% weight loss
2. Adding on anti-obesity medication to diet and exercise: 5-12% weight loss
3. Medical supervision (working with an obesity medicine doctor) added on to diet, exercise and medication: 10-15% weight loss
