At our office we make sure we throughly clean the bone after a
Tooth is pulled. A clean bone has a very clear consistency with nothing mushy stuck to the walls.Something like what the picture below shows. We first start by studying the bone topography on a cbct scan and analyze which part of the socket looks infected, then we clean those with hand instruments and laser,then insufflation the sockets with ozone gas and prp(platelets rich plasma) & prf(platelets rich fibrin).

Patients immediately notice the difference in the energy after the surgery.Or gain the movement or cessation of dull ache they have been experiencing for years.

C reactive protein: C-reactive protein (CRP) is a protein made by the liver. The level of CRP increases when there's inflammation in the body. A simple blood test can check your C-reactive protein level.

The hs-CRP test can help show the risk of getting coronary artery disease.Your health care provider might order a C-reactive protein test to:

Check for infection.

Help diagnose a chronic inflammatory disease, such as rheumatoid arthritis or lupus.

Learn your risk of heart disease.

Learn your the risk of a second heart attack.

A note about CRP tests for heart disease

A high level of hs-CRP in the blood has been linked to an increased risk of heart attacks. Also, people who have had a heart attack are more likely to have another heart attack if they have a high hs-CRP level. But their risk goes down when their hs-CRP level is in the typical range. The test doesn't show the cause of inflammation. So it's possible to have a high hs-CRP level without it affecting the heart. An hs-CRP test may be most useful for people who have a 10% to 20% chance of having a heart attack within the next 10 years. This is known as intermediate risk. A health care provider can determine your risk using tests that look at your lifestyle choices, family history and overall health.

Chronic Inflammation & CRP:

If inflammation lasts too long, it can damage healthy tissues. This is called chronic (long-term) inflammation. Chronic infections, certain autoimmune disorders, and other diseases can cause harmful chronic inflammation. Chronic inflammation can also happen if your tissues are repeatedly injured or irritated, for example from smoking or chemicals in the environment. Most dental infection fall into this category. A CRP test can show whether you have inflammation in your body and how much.

CRP &Dental foci inflammation:

Low grade chronic inflammation, CAP(Chronic apical periodontitis) leisions surrounding dead/non vital/ failing root canal teeth can all be poteintial sources of Chronic Inflammation in the body which can be related to hihg CRP numbers. Activation of T cells(which are a part of our immune system) due to dental inflammation in systemic inflammatory processes (like- Cancer, Heart disease, Arthritis(substance P), autoimmune diseases, neurodegenearative diseases like parkinsons, and Alzhiemers disease) causing elevated levels of Interleukin-6 (IL-6) and chemokine ligand 5 (CCL5), also known as RANTES (regulated on activation, normal T cell expressed and secreted). Higher the RANTES Score higher the incidence of chronic oxidative stress related diseases.

Dental Revisions & reduction in CRP score:

A dental revision is a process in which potentially toxic materials and infected teeth are removed and replaced with biocompatible materials specific to each patient’s individual immune system whihc can be tested via Biocompatibility tests. Dental Revisions may include removal of mercury fillings, nickel crowns, old metal crowns and bridges, infected or failing root canal treated teeth, dead/infected teeth, impacted wisdom teeth with silent infections and thorough debridement of bony sites. We use immune boosting protocols that optimize healing response.

We higly encourage our clients to work with team of collaborative providers which include- Functional medicine Doctors, naturopaths, Also getting complete blood work, and functional panels, theromogram reports. We also reccomend to follow up those reports after dental work to see the progress in their systemic health.

Beth had reduction in her CRP level from 3 to 1 without any other changes in her lifestyle other than dental revision performed at Prime Integrative Dentistry. Other changes noticed by her includes:

Dr. Mirkal, along with Dr. Gray (Doctor of Osteopathy), can perform Guided Releases (frenectomies) at our office after having worked with Melinda Rogers(Trailblazer Myofunctional therapy).
Dr. Gray screens patients for needed body work and he provides insight on which group of muscles exhibit tension. Dr. Mirkal correlates those group of muscles exhibiting tension to fascia attachments in the mouth. With the help of a CO2 laser, Dr. Mirkal release those adhesions.

The International Journal of Molecular Sciences has come up with an article on Root Canals and the toxicity associated with them. Lymph nodes from our oral cavity ultimately drain into the Breast Lymph nodes. This Article explores in depth the correlation between Breast Cancer and Latent inflammation arising from anerobic bacteria( bacteria that does not need oxygen to thrive) & the toxins released from them. This inturn is responsible for elevating the Rantes Score which is the prime indicator of Chronic oxidative stress in the body(Elevated T Cells in the body). Cancer is the most common example of Chronic Oxidative stress associated with Elevated Rantes Score.

CAP Leisions and Chronic Oxidative Stress of Dental Origin & Alternatives to treat them:

When dental infection most commonly of dental decay origin spreads past the dentin of the tooth and enters the lving tissues it leads to irreversible infection that further leaks into the bone structure forming soft cycst like chronic infection usually referred to as 'CAP Leision'(Chronic Apical Periodontitis).Anything that is non-vital or dead is usually energetically a break in the circuit. This is the reason why people who have multiple infected teeth complain of chronic fatigue, insomnia as some of the presenting symptom. Any non-vital part of the body is the breeding ground for gram negative anaerobic bacteria and the toxins from this low grade chronic inflammation(silent inflammation) activates the immune system. People with multiple root canal treated teeth usually present with some form of autoimmune condition on their health history.

A safer more sterile alternative would be to clean out that infection, and treat the area with ozone, laser & PRF with replacing the tooth with biocompatible CCeramic Implant.

Anterior Cross-bite correction:

These are mid treatment progress results of primary/mixed dentition cases.Treating cross bite early on and not waiting till they turn 12 years is key. In both these cases we have uses RGGA appliance therapy, that works very well for 6-11 year age group.

RGGA (Removal Growth Guidance Appliance):

Craniofacial development in children starts early on and It is critical to intervene as early as possible to take advantage of the chance to manipulate normal skeletal growth occurring in children. By the age of four, 60% of facial growth is complete. By the age of six, 80% of facial growth is complete. By the age of 11-12 90% of facial growth is completed and the second premolars have erupted. It is important to introduce orthotropic, orthopedic interventions which harness the ability to remold skeletal growth patterns. RGGA appliance is a full fledged functional and TMJ appliance in kids aged between 6-9 years. Many children in this age group experience headaches, and TMJ symptoms but rarely verbalize it. Other symptoms to watch for:
Problems with teeth alignment – crowded or crooked teeth
Bite problems
Teeth don’t meet correctly
Underbite or overbite
Facial asymmetry
Frequent headaches
Snoring
Open mouth breathing
Short attention span/easily distracted
This appliance will help remold the downward growth vector to a more horizontal growth( long flat face versus more fuller attractive forward face). Orthodontic interventions done later on will be relatively straight forward and less invasive after RGGA.
