Psychologists Maytal Eyal and Bridget Freihart, in an essay for Time Magazine, discussed the postpartum care, or lack thereof, provided to new mothers in the United States.
The reality of just one postpartum visit, without essential education and guidance for navigating the fourth trimester and beyond, is unfortunately quite common. ✨Case Study Time!✨
This past weekend, we at HVPT once again had the pleasure of hosting Lynn Schulte of and taking her phenomenal 3 day course: “Holistic Treatment of the Pregnant Body”. This beautifully structured hands-on course gave us the opportunity to evaluate and treat 4 lovely moms-to-be at the end of Sunday’s session to really pull it all together.
Shared with permission, the above photos show a “before and after” look at M.J.’s beautiful pregnant belly. M.J. is a 30 y/o female, currently 23 weeks gestation with her first pregnancy. She came to our clinic complaining of left sided hip pain after sleeping, lower back pain at the end of the day, and severe restless leg syndrome at night that often kept her from getting much needed rest. Further questioning revealed that her iron levels were not “significantly low” and her providers did not recommend an iron supplement at this time.
As you can see, her belly button and linea nigra (a dark, vertical line that appears on the belly during pregnancy) was pulling towards the left in the before photo.
After performing gentle and respectful hands on manual techniques learned in this course, we were able to improve balance in M.J.’s pelvic bones and uterine ligaments as well as mobilize and balance her respiratory diaphragm. As you can see in the “after” photo, her belly button and linea nigra are more midline and M.J. stated “I feel like I can finally take a full breath!” when she stood from the table. Following up with her Monday morning, M.J. stated “I feel great today and last night was the first night in the last 6 weeks or so when I had NO restless legs at all.”
We had an incredible weekend and are so excited to share everything that we learned with our pregnant and postpartum clients!

Reposted The gold-standard of treatment for lichen sclerosus is topical steroid. Unfortunately, this medication is often prescribed with very little instruction on how to use it, which leads to it “not working” or side effects
Also, just applying medication alone is usually not enough. Treatment works best when it is holistic and based upon how the body works.
Here are some helpful adjuncts that can help with remission:
Diet- although there is limited research, limiting or eliminating gluten, oxalates, dairy, and excess sugar may help with symptoms, especially itch.
Hormones- lack of hormones do not cause lichen sclerosus, but may "bring out" symptoms. It is important to treat genitourinary syndrome of menopause (GSM, formerly vulvova**nal atrophy) and hormonally-associated vestibulodynia, if present, with local low-dose estrogen (and possibly testosterone), which is safe and effective.
Sitz Baths/Soaking- it is essential to soak prior to application of topical steroid, especially when lichen sclerosus is active (meaning the skin is thickened/lichenified). Some people add sodium bicarbonate (baking soda) or sodium tetraborate (borax) and find that this helps with symptoms and fusing. However, we have no research on this and the British Association of Dermatologists put out a statement (updated February 2020) against using sodium borate due to safety concerns.
Vulvar moisturizer- locking in moisture can be very helpful to heal lichen sclerosus skin. In addition, this counteracts any dryness that occurs related to GSM, soaking, and topical steroids. It's important to pick a moisturizer that has non-irritating ingredients.
Pelvic floor physical therapy- people with LS may also have tight and weak pelvic floor muscles, so this is an important part of treatment if there is genitopelvic pain
Manage stress- stress and anxiety are major triggers. Managing these with deep breathing, meditation, yoga, apps, talk therapy, etc. can be very helpful.
Dilator therapy- if there is scarring and tearing at the va**nal opening, dilators with lubrication may be used to help smooth

Chronic vulvova**nal pain is extremely common, impacting 1 in 4 people with vulvas! Yet it’s also severely underfunded and under-researched. Federal funding for vulvodynia amounts to just 33 cents per affected patient. Our pain and our lives are worth more than 33 cents apiece.
Many of us have spent years, if not decades, searching for pain relief and care. We, as patients, are the ones who pay the price for the gap in research.

Vaginal dilators. So. Many. Options.
Which one to choose??
The answer you may not want to hear: It depends.
It depends on your own situation, your medical history and most importantly your specific personal goals.
In this case our patient had a history of radiation therapy due to vulvar and a**l cancer. Her goals for pelvic physical therapy was to resume $exual activity and to have
penetrative $ex without pain.
Radiation therapy to all tissues causes scar formation and makes healthy tissues fibrous, more stiff and less elastic (Semin Plast Surg. 2021 Aug; 35(3): 181–188.) Pelvic Radiation therapy produces the same results to va**nal/a**l tissue: thinned va**nal mucosa, lack of lubrication, scarring. (Medicina. 2021; 57(4):336). Basically, it leads to a shorter, dryer, less elastic va**na. The medical term is va**nal stenosis. In our patient’s case, it’s pelvic radiation induced va**nal stenosis.
Our patient brought in the dilators she had used with unsuccessful results. In our opinion, she did not receive proper instruction and training in using the dilators in her home program which caused her to give up. She also brought in an expanding va**nal dilator from She also did not use it often as she did not find it helpful.
In addition to our external and internal manual therapy treatments, we educated her on the importance of using over the counter va**nal moisturizers such as, Restore Moisturizing Vaginal Gel from , to help address va**nal dryness.
She's on level 2 va**nal dilator now after 3 weeks of her home stretching program. Slow and steady wins the race.
More importantly, she was successful and confident in using her dilators after proper instruction.
Vaginal dilators are user specific. One size does not fit all. You cannot purchase one,
"Stick it in", breathe and hope for the best. ❌

Fun filled holiday capped off by a weekend of education. Thrilled to attend this inaugural internal pelvic floor techniques course taught by Olivier Bazin and hosted by the Barral Institute of Denmark.
As a pelvic health physical therapist, manual therapy techniques, specifically visceral manipulation, are an essential component of treatment. It's all about knowing your anatomy, organs, muscles, nerves, blood vessels, fascial connections...All of it...and what structures maybe contributing to the presenting symptoms.
Sometimes, where you feel your pain or discomfort is not the source of your pain or discomfort.
Visceral manipulation gives the body the information it needs to assist with movement, mobility and its journey to heal.
Cannot wait for Part 2 of this course.
