This is how medical science does its amazing feat of ameliorating PAD symptoms of pain on walking by improving blood flow by opening up clogged blood vessels in the legs by angioplasty. In this case Posterior Tibial artery was cleared of the atherosclerosis.
Our student Chaitra Rao passed CCI boards and is now a Registered Cardiac Sonographer.
AAA Endovascular graft on 73 yes old male patient of mine(DrKhan). Completely Endovascular. No cutting involved. Femoral artery site’s closed by Perclose device. Patient start ambulating in 2 hours and go home same day. Few years ago it used to be 6 hours long high risk complicated surgery with 7-10 hospitalization .
Thanksgiving at NDRA
37-year-old developed extensive DVT of left leg after Iliac stent due to May Thurner syndrome. Ultrasound showed extensive blood clot of Iliac Vein, SFV and Popliteal vein. Patient brought to hospital, via popliteal vein excess thrombectomy (Pneumbra device)was performed by Dr Akram Khan. Blood flow was completely restored, and patient went home next day.

Why are our legs so important? Imagine all the things we do all day: could they have been possible without our legs? Not at all!
That is why we celebrate Peripheral Artery Disease (PAD) in the month of September every year to spread awareness. Did you know that roughly 8.5 to 12 million Americans over the age of 40 suffer from this deadly disease and most don’t even know they have PAD.
Why PAD happens:
Narrowing of arteries in pelvis and lower limbs due to plaque (atherosclerosis) build up leading to poor blood supply causes pain and other symptoms of PAD. This disease is progressive and can lead to significant morbidity and mortality as we age.
Symptoms of PAD:
1. Intermittent Claudication: cramping, pain or just fatigue or discomfort during walking and climbing stairs which get relieved with rest ( within 10 minutes) progressing to rest pain.
2. Wounds or sores on lower limb which don’t heal.
3. Less growth of hair or nails, shiny skin.
4. Legs feel cooler one from another.
5. Discoloration of legs or feet.
6. Tingling or burning in legs.
7. Erectile Dysfunction.
8. No symptoms
Confusion over leg pain:
We might think its just part and parcel of getting older. Get the pain checked out. Examination and investigations by your doctor can narrow down the diagnosis which may be:
1. Arthritis of any joint: hip, knee, foot arthritis giving localized persistent grinding pain on moving joint, relieved by analgesics.
2. Baker’s cyst: persistent swelling and pain going down from behind knee.
3. Nerve root compression: Sharp Lancinating Pain radiating down the leg often at rest in people with back problems.
4. Veinous pain: tight bursting pain and swelling more in legs, more after walking, relieved by elevation.
5. Chronic Compartment syndrome: tight bursting pain after much exercise in athletic people.
These are only a few differentials and barely touch few aspects of the disease needing expert consultation.
Risk Factors:
1. Smoking
2. Hypertension
3. Diabetes Mellites
4. Increased Cholesterol
5. Obesity
6. Sedentary lifestyle
7. Ill-informed dietary habits
Diagnosis:
Doctor after taking detailed history and examination may ask for investigations to confirm his suspicions.
Signs Of PAD:
1. Color change of lower limbs
2. Temperature difference
3. Leg numbness
4. Leg weakness
5. Weak or no pulse
6. Vascular bruits
7. Shiny skin
8. Sores or ulcers on legs or feet
Investigations:
1. Resting Ankle Brachial Index (ABI): Doppler Ultrasound checking difference in upper and lower limb Systolic Blood pressure may be all that’s required to make the duagnosis
2. Duplex Ultrasound.
3. MRI
4. CT Scans
Treatment:
Its important to appreciate that PAD is a chronic disease needing not only accurate diagnosis and management but also long term followup and close monitoring for best results.
Treatment options broadly categorized are:
1. Life style Changes: which includes:
a. Quitting smoking
b. Healthy dietary adjustments
c. Regular exercise
d. Weight management
2. Medications: controlling symptoms and predisposing diseases
a. Blood thinning medications
b. Blood pressure medications
c. Cholesterol lowering medications
d. Sugar controlling medications
3. Minimally Invasive Procedures: like angioplasty to remove plaque and opening arteries
4. Surgery: in severe and neglected cases open surgery to bypass complete obstructions

North Dallas Research Associates is located 30 miles north of Dallas in the fast-growing McKinney area, which is part of Collin County in Texas.
