Chikungunya Vaccine Now Available 02/19/2024

We are pleased to inform you that the New York Center for Travel and Tropical Medicine is now providing the IXCHIQ Chikungunya virus vaccine. This vaccine is recommended for individuals aged 18 and above who are at increased risk of exposure to Chikungunya virus (CHIKV) and prevents disease caused by this virus.

The CHIKV-carrying mosquitos are present in tropical and subtropical regions of the world including Africa, South and Southeast Asia, and parts of the Americas, posing the highest risk of infection. Because of climate change, we are seeing increased risk in popular tourist destinations such as Puerto Rico, other Caribbean islands, as well as Central and South America. Chikungunya is a particularly serious infection because it may lead to chronic arthritis symptoms even in young adults.

🚨 Travel Alert for those traveling to Denmark:

According to Denmark's Ministry of Health, more than 290 laboratory-confirmed cases of pertussis per month, a significant increase over average incidence, are being reported. More than 680 cases have been reported since May 2023 throughout the country. The outbreak has yet to peak. Many adults lack immunity to pertussis even if they completed a normal childhood series with a tetanus/diphtheria/pertussis vaccine. All adult travelers should receive Tdap vaccine. Parents of infants who have not yet completed 3 doses of a pertussis-containing vaccine may want to consider postponing travel until the infant has received the third dose.

Vaccine Update! 💉

The recommendations for pneumococcal vaccination have been changed to now include children with chronic kidney disease (regardless of stage), chronic liver disease, moderate- or severe-persistent asthma, or who are on maintenance dialysis. PCV20 (Prevnar 20; Wyeth/Pfizer) has now been incorporated into the recommendations as follows:

- All children aged 2-23 months (routine): Administer a dose of either PCV15 (Vaxneuvance; Merck) or PCV20 according to currently recommended PCV dosing and schedules.

- Healthy children aged 24-59 months and children aged 24-71 months with underlying conditions (that increase risk of pneumococcal disease) with an incomplete PCV vaccination status (catch up): Administer a dose of PCV15 or PCV20 according to currently recommended PCV dosing and schedules.

- Children aged 2-18 years with any risk condition and who received all recommended doses before age 6 years: If ≥ 1 dose of PCV20 was used, no additional doses of any pneumococcal vaccine are indicated; this recommendation may be updated as additional data become available. If PCV13 (Prevnar 13; Wyeth/Pfizer) or PCV15 (no PCV20) was used, administer a dose of PCV20 or PPSV23 (Pneumovax 23; Merck) using previously recommended doses and schedule.

- Children aged 6-18 years with any risk condition who have not received any dose of PCV13, PCV15, or PCV20: Administer a single dose of either PCV15 or PCV20. If PCV15 is used, it should be followed by a dose of PPSV23 at least 8 weeks later if not previously given.

🛑 Travel update for those traveling to :

According to the Philippines' Department of Health, more than 1,000 cases of leptospirosis (including 85 deaths), a significant increase over average incidence, were reported from January through mid-March 2023 throughout the country, mainly in Western Visayas and Cagayan Valley regions. The outbreak appears to be past peak. Travelers going to the Philippines who anticipate activities with extensive outdoor exposure (e.g., hiking, biking, swimming, or rafting) should consider prophylaxis with 200 mg of doxycycline once per week.

Hajj and Umrah are rapidly approaching! Proof of meningococcal vaccination (issued at least 10 days but not > 5 years prior to arrival, with a quadrivalent meningococcal conjugate vaccine) is required for all travelers aged > 1 year arriving for the purpose of Umra or Hajj pilgrimage or seasonal work in Hajj/Umra zones. The certificate must clearly state conjugate for 5-year validity to apply.

🚨 Travel Update: Malaria has been reported in Costa Rica:

According to Costa Rica's Ministry of Health, 60 cases of Plasmodium falciparum malaria have been reported since January 1, 2023, in Limón Province. More than half of these cases have been reported since March 26, 2023. Travelers should observe insect precautions in areas with any level of transmission.

❗Travel update: Bolivia has recently seen an increase in pertussis.

According to Bolivia's Ministry of Health, more than 260 cases of pertussis (mainly in unvaccinated children aged ≤ 5 years), a significant increase over average incidence, have been reported since November 2022, mainly in Santa Cruz Department. Many adults lack immunity to pertussis even if they completed a normal childhood series with a tetanus/diphtheria/pertussis vaccine. All adult travelers should receive Tdap. Those who received Td or TT for their most recent booster should receive an immediate dose of Tdap, regardless of the interval since the last tetanus dose. Parents of infants who have not yet completed 3 doses of a pertussis-containing vaccine may want to consider postponing travel until the infant has received the third dose.

🦚 Travel Advisory for those traveling to , for !

Approximately 1 million persons are expected to attend Rio Carnival during February 17-25, 2023. Events will take place throughout the city of Rio de Janeiro. Currently, fully COVID-19–vaccinated travelers may enter Brazil without restrictions. Daytime insect precautions are especially important for travel to Brazil. While there is no requirement for pre-entry yellow fever vaccination, there is risk with travel to this area and it is recommended to receive the vaccine beforehand.

📣 TRAVEL UPDATE: CHOLERA IN KENYA📣
According to WHO's regional office and international health authorities, more than 420 cases of cholera per week, a significant increase over average incidence, are being reported, mainly in Garissa County (including in a refugee settlement in Dadaab) and central counties. More than 1,900 cases have been reported since October 8, 2022, in 11 of 47 counties, mainly in Garissa, Kiambu, Machakos, and Nairobi City. Risk to travelers is minimal. Cholera vaccination (not currently available in the US) should be considered for aid and refugee workers and medically vulnerable persons going to affected areas and for travelers visiting friends and relatives in these areas. Travelers, regardless of vaccination status, should observe strict food and beverage precautions and hand hygiene (frequent, thorough handwashing) and carry empiric antibiotic therapy (azithromycin) and oral rehydration solution (ORS) for self-treatment if any moderate-to-severe diarrhea develops. Travelers who develop watery diarrhea should ensure adequate intake of clean water and avoid high sugar (high osmolality) beverages. In case of severe, watery diarrhea, oral rehydration (ideally with an ORS) should be considered if oral fluids are tolerated.

❗Travel Update: Monkeypox has been circulating the news for months now. Here's the latest data:

More than 81,500 laboratory-confirmed and suspected cases (including 47 deaths) have been reported since May 13, 2022, in the US (> 29,300 cases and 15 deaths), Brazil (> 10,000 cases and 12 deaths), Spain (> 7,200 cases and 3 deaths), France (> 4,100 cases), Colombia (> 3,800 cases), the UK (> 3,700 cases), Germany (> 3,700 cases) and Mexico (> 3,200 cases), and 97 additional nonendemic countries. Cases have mainly been reported in the European and Americas regions (98.3%). An additional 864 laboratory-confirmed cases (including 9 deaths), separate from the global outbreak, have been reported since January 2022 in 9 endemic African countries (mainly Democratic Republic of the Congo and Nigeria) and in 1 nonendemic African country (Benin; cases mostly imported from Nigeria).

Community-based transmission has occurred in many countries among persons having close, sustained physical contact with persons who have monkeypox. MSM (median age = 34 years) remain an important demographic in the outbreak, representing a majority of cases, and 52.0% of persons for whom HIV status is known have HIV. However, sporadic cases have been reported in household members, nons*xual contacts, heteros*xual contacts, and children.

Travelers should avoid close contact with persons with symptoms consistent with monkeypox; observe hand and respiratory hygiene; reduce the number of s*xual partners; reconsider s*x with new partners; observe safer-s*x practices; and abstain from s*x when partners are ill (including any rash). Vaccination is only recommended for known and presumed contacts identified through investigations. Monkeypox should be considered in any traveler presenting with compatible illness within 21 days after potential exposure.

🛑 TRAVEL UPDATE: MEASLES IN INDIA 🛑

According to WHO, more than 9,400 cases of measles, a significant increase over average incidence, have been reported since January 2022, throughout the country, mainly in Maharashtra State (> 6,500 cases; including Mumbai). All persons aged ≥ 12 months born in 1957 or later without history of disease or of 2 countable doses of live vaccine at any time during their lives should complete a lifetime total of 2 doses of MMR vaccine (spaced by at least 28 days). All infants aged 6-11 months should receive 1 dose of MMR vaccine.

❗Travel Update for those planning on traveling to ❗

According to WHO's regional office, 4 laboratory-confirmed cases of yellow fever (YF), a significant increase over average incidence, have been reported since September 1, 2022, in Agadez, Dosso, Tahoua, and Zinder regions. The most recent date of symptom onset was August 14, 2022. YF had not been reported in Niger since 2011.

🚨TRAVEL UPDATE FOR ANYONE TRAVELING TO THE LIMA, PERU METROPOLITAN AREA🚨

According to Peru's Ministry of Health, more than 115 cases of methanol poisoning from counterfeit/adulterated alcohol have been reported since August 29, 2022, in the Lima Metropolitan Area (including Callao Region), mainly in Comas (19 cases), Carabayllo (19 cases), and Puente Piedra (17 cases) districts. Travelers should only consume alcohol from original, sealed, labeled, and branded containers in well-known, reputable establishments and avoid any drinks containing methanol.

The New York Center for Travel and Tropical Medicine, in partnership with The Centers for Disease Control and Prevention (CDC), is looking for volunteers who are planning to travel internationally, to participate in a research study to help prevent traveler’s diarrhea from occurring.

Please note:
We have the capability to enroll participants offsite. We will need an e-signature and will mail all study materials needed (participants MUST be located within the US).

People who qualify:
• Are 18 years of age or older
• Plan on traveling internationally for at least 7 days with a maximum stay of 21 days
• Traveling to Africa, Southeast Asia, or South-Central Asia
• Traveling from the United States to an eligible country

There are other criteria that you will need to meet to qualify, which our study team will discuss with the interested participants. There is no cost to participate in this clinical research study. Qualified participants may receive a maximum compensation of $300 for their time.
