CervicoThoracic Diaphragm course is complete! We had an hour long review of the different facets of the CT diaphragm, including: the Stylian portion, Bourghery portion, the important unorganized fascia of the neck…to name a few. The Sauna… so much to say. As a student of traditional osteopathy with Guy Voyer DO I took my first sauna course in 2012. The physiological benefits are many and safety and proper progression is paramount. The benefits, in brief: brown fat stores, heat shock proteins, , cleaning the Extracellular water out, heart rate variability, training the Heart Rate (HR) variable of Cardiac Output (CO=SVxHR), and many many more. Stay tuned as I take a group of patients through some baseline labs, fasting, dry sauna, etc over the next couple of months!

Complexity in Clinical Reasoning. My goal is to take you on a journey of my own Patellofemoral case. Exam findings, imaging, treatment, specific exercise, modalities and progress notes. I’ve had PF pain in my R knee one other time in my life. When my oldest daughter was born, and gym/bike/surfing time was difficult to come by. Once I started training, doing MyoFascial Stretches, specific segmental proprioception and Eldoa it all went away. That was 5 years ago. Since then, I’ve trained my lower body, upper, trunk etx segmentally and functionally at least 3x per week. Nothing changed as of recent, except I started Mtn biking again to gain back my mitochondrial health/ aerobic capacity…pg1

ICYMI. 3rd and final question from last week’s webinar . Doc, hey did you mention the dissection experience and the type of chemical preparation in the specimen? Answer: The chemicals (or lack thereof) used in the preservation of the specimen is directly related to your viewpoint of the human body that you treat or rehabilitate. The formaldehyde (formalin) concoctions used, dehydrate the fascia/tissues that we are so intrigued to study. If it’s dry, you the therapist thinks it’s dry…but it isn’t…

Answer: the basics are a) movement of blood and lymph are accelerated w/out any expenditure b) the movement of extra/cellular fluids gets equalized throughout the body c) the plasma and CSF get renewed faster… and many more! In case you missed it! A little Q&A from yesterday’s webinar with 1 of 3 questions posed. Doc, how do I treat differently to better respect the fluidity of the the body/ fascial system? Answer: perhaps try and use less force at end range on a restricted joint and instead use 50 reps of of a gentle rhythmic oscillation, adding decoaptation whenever possible OR Move your client or patient in a pool, fully suspended, using simple movement patterns related to the affected kinetic chain OR better yet, come and study OsteoArticular Pumping in February 2024!

Now that I have an extra minute, let me further explain my last post.
This image comes from a book by Igor Burdenko PhD. Who Bill Knowles studied with, early in his career. This pyramid reflects the importance of 6 different qualities of an athlete and patient. The ⭕️ show the main focus of training in the US. The lack of foundational balance/coordination etx can lead to a boat load of pain, dysfunction and lack of performance (although it gives us a never ending supply of athletes). Specifically hamstring injuries. The success rate, IME, not only has to do with a lense for the deep structure quality but a lot of times the lacking of the inferior 4 qualities in this pyramid. How many athletes with with HS injuries are explosive and fast and strong?… Have a great weekend.
