Secondary Insurance is Broken 05/07/2019

New Blog Post: Secondary Insurance is Broken
I hate secondary insurance coverage. It is a total scam on the consumer and should be illegal as it currently exists. I used to think Dental Insurance was the worst medical insurance product you could buy. I think probably now that the worst policy you can buy is a commercial policy when there is already another policy in force. It costs full freight, but the benefits are a fraction of what they would be if it were the only policy in force.

Hello Academic OB/GYN Fans! My new practice Northwest Endometriosis and Pelvic Surgery has a new page "Northwest Endometriosis and Pelvic Surgery - Nicholas Fogelson, MD". If you would like to get updates on new blog posts and messages from me, please like that page.

I was hoping to just transition this page over being the page for the new practice to preserve the almost 3000 people following it, but it seems that Facebook does not allow this. So please like the new page!

I would also help if you like and/or share this post and leave a comment so that other followers will be more likely to see it and make the transition!

My beautiful wife is a great cook and a healthy eating aficionado, and recently has started a health coaching consultancy. Check out Wild Lilac Wellness for great cooking and health tips. Not endometriosis specific, but I often recommend an low inflammation diet to my patients and Zarya’s recipes are mostly in that category as well.

Hello Academic OB/GYN fans and readers - I wanted to let you all know that I do not post a great deal on Academic OB/GYN Facebook anymore, preferring my professional facebook profile "Nicholas Fogelson - Physician". If you want to follow what I'm posting please like that profile and you'll get occasional updates from me. Academic OB/GYN started in 2005, and now in 2018 I'm no longer in academia, nor am I an OB/GYN anymore. My primary practice is in minimally invasive gynecologic surgery with a focus in endometriosis care. So if you're not a friend of "Nicholas Fogelson - Physician" and want to continue to get updates, please send a request and I'll accept it.

Obturator Internus Muscle Endometriosis with Nerve Involvement: a Rare Clinical Presentation 02/23/2018

Was just reading JMIG and found an amazing case and video:

What is amazing about this is that the pt has an endometrioma within the obturator internus muscle and NO PELVIC ENDOMETRIOSIS. One usually believes that cases of very deep retroperitoneal endometriosis come from extension of pelvic disease, but in this case this is clearly not the case. In fact, the pelvis is entirely clean and the surgeon had to cut into a pristine looking pelvic sidewall to reach the very deep lesion within the obturator internus muscle. Fascinating and also beautiful technique.

Being denied an abortion may lead to poverty 01/21/2018

Being denied an abortion may lead to poverty It's linked to long-term consequences.

Just saw a patient today with the clearest case of abdominal wall endometriosis you will ever see. What is fascinating about it is that she saw several doctors previously, all of which did all kinds of things before making the diagnosis, including putting a needle in the thing to prove that it was what it was. Here's the thing. If there is a mass in a previous surgical scar, particularly a cesarean scar, and it hurts really bad every month, its endometriosis. No biopsy is required. Just needs to be cut out and the resulting hole needs to be fixed. She is posted for surgery now and will be cured of this shortly.

Abdominal wall is endo is different than peritoneal (traditional endo). With peritoneal endo some women get dramatically better from surgery and some struggle even though they have as good a surgery as can be done. But with abdominal wall endo surgery works very reliably and completely as long as the lesion is thoroughly removed.

Drug Companies Make Eyedrops Too Big, And You Pay For The Waste 10/18/2017

Another example of how profit motives undermine simple changes that would save healthcare dollars. When profit is the motive in medicine, there is every incentive for healthcare to be more expensive.

Drug Companies Make Eyedrops Too Big, And You Pay For The Waste When eyedrops dribble down your face, it's not your fault. Drugmakers have long known that their drops of medicine exceed the capacity of the human eye. Why didn't companies make the drops smaller?
