Transitions LifeCare - NC

Founded as Hospice of Wake County in 1979. Serves 8 counties including Wake. Oldest, largest and most experienced end-of-life organization in the area. Why?

"Hospice of Wake County, Inc. Becomes Transitions LifeCare April 30, 2014"

For several years Hospice of Wake County has offered more than “hospice” care and has served patients well beyond “Wake County.” Though we changed our name to Transitions LifeCare, our mission and vision are unchanged and we remain a local, non-profit serving patients regardless of their ability to pay. Crafted by Carrboro sculptor Mike Roig, the Cosmic Post, located on our campus, is a functional sculpture that allows you to “mail” correspondence to your loved ones that no longer have an earthly address. The letter is then burned, creating tendrils of smoke, symbolizing a continued connection with your loved one and carrying the message to its intended recipient. 💌

If you would like to use the Cosmic Post, call 919.719.7199 or speak with your bereavement counselor. 💟

We love our volunteers! Put on this week by Janet and Janie, tea time is a cherished tradition that takes place two times a week at our hospice home featuring warm homemade cookies, muffins, and more delicious sweets. It also gives our families and patients time together to share cherished moments. We wish you could smell the warm cookies through the screen… Such a special treat from our amazing volunteer team! 💜🍪🫖
