Transitions Kids

Transitions Kids compassionately cares for children, families, and communities significantly impacted by illness or loss.

Transitions Kids is an interdisciplinary team of Pediatric Palliative Care Specialists (PPCS) who provide support to families. We empower families to set and advance their goals based on a collaborative exploration of their values. We support family enrichment, self-care, and celebration, especially during difficult times, and we extend emotional and spiritual support to the entire community of those impacted by illness or loss.

Transitions Kids received a $2,000 grant from the Ladies of Philoptochos Society at the Holy Trinity Greek Orthodox Church. This grant supports our hospice, palliative care and bereavement services for kids and their families. Since 2016, the Ladies have raised more than $54,000 for Transitions LifeCare and our Kids program! As we continue through November, we will be sharing statements that can be invaluable to someone grieving. These statements aim to provide comfort, empathy, and support to individuals experiencing the profound loss of a loved one.

My name is Alex Stephens, and I am a Pediatric Palliative Care Specialist with Transitions Kids! I have been a nurse for four years now, with a background in Pediatric ICU and Pediatric Cardiology nursing before I worked with TK. Meet the Transitions Kids Team!๐ŸŒŸ
My name is Emily Parker, and I am one of the Pediatric Palliative Care Specialists with a background in Nursing with Transitions Kids. I started part time about 5 years ago but loved it so much I upgraded to full time. Recently I became one of the preceptors to help educate new employees and student learners about pediatric palliative and hospice care. Prior to my role with Transitions Kids, I worked in pediatric bone marrow transplant for almost 20 years. My husband and I recently celebrated our 17th anniversary. We have 3 kids together, a son and two daughters. When I am not at work you can likely find me on the soccer sidelines, doing laundry, or reading a book (please give me any recommendations). We enjoy road tripping together and have visited every state east of the Mississippi River. A couple of our favorite places are Acadia and Sleeping Bear Sand Dunes. We are hoping to explore farther west soon.

My name is Jaclyn McKinley! Iโ€™m very new to the TK team, but Iโ€™m excited to be here. Iโ€™m a full-time flight nurse that transports babies and kids and I enjoy helping kids and families through TK when Iโ€™m not in the helicopter. I have a couple of fur babies of my own - Harper is a golden retriever and Elsa and Bella are kitties. I enjoy spending time with my family and friends and being outside! My favorite season is fall and my favorite color is purple!
