CDC Antimicrobial Resistance Threats in the United States, 2021-2022

The CDC recently provided an update on the burden of seven antimicrobial-resistant pathogens typically found in healthcare settings from 2021-2022 in the United States. The seven antimicrobial-resistant pathogens the new data follows were Carbapenem-resistant Enterobacterales (CRE), Carbapenem-resistant Acinetobacter, Candida auris (C. auris), Methicillin-resistant Staphylococcus aureus (MRSA), Vancomycin-resistant Enterococcus (VRE), Extended-spectrum beta-lactamase (ESBL)-producing Enterobacterales, and Multidrug-resistant (MDR) Pseudomonas aeruginosa. Previously the CDC reported these pathogens increased in 2020 in the United States and the new data compares data from 2021 and 2022 to data from 2019.

The new data for the burden of six antimicrobial-resistant pathogens reveal a combined 20% increase during the COVID-19 pandemic compared to the pre-pandemic period. In 2022, the change in rates or number of infections for MRSA is stabilized compared to the increase for the six other pathogens. Antimicrobial resistance remains a public health threat in the United States and additional action needs to be done to combat antimicrobial resistance.

The CDC is implementing a new electronic reporting format in 2025 that will report on estimates for at least 19 antimicrobial resistance threats and an update on the U.S. burden of antimicrobial resistance. Once the new format is implemented, the CDC will release new estimates for the burden of these threats at least every two years.

Last month, an article was published in the JAIDS Journal of Acquired Immune Deficiency Syndromes secondary to the work done with the California Pharmacist PEP & PrEP Survey.

CSHP would like to celebrate this publication and the two CSHP leaders involved with this article, Dr. Loriann De Martini and Dr. Jerika Lam.

This study evaluated the potential for LAI-PrEP provision in pharmacies through a mixed methods study of pharmacists in California, where Senate Bill 159 enables pharmacists to independently provide oral PrEP.

CDPH Recommends Students Complete Routine Vaccinations Ahead of New School Year

The California Department of Public Health (CDPH) reminds Californians with children in school to stay up to date on routine vaccinations and ensure their students receive the required recommended vaccinations before the new school year. It is required by California law for students enrolled in public and private elementary schools, secondary schools, and licensed childcare centers to receive age-specific immunizations before attending.

Vaccinations help protect against and prevent contagious diseases. Many preventable diseases such as the human papillomavirus (HPV) can lead to some forms of cancer if infected early on in life. HPV vaccination can help protect against HPV infection-related cancers. Children as young as 9 years ago are recommended to receive the HPV vaccine and schools in California are required to notify families of 6th graders about the HPV vaccine.

Families can check with the CDPH’s resource pages on the required immunizations to attend schools in California. Pharmacists in California can recommend, remind, and administer vaccines to students based on the CDC’s immunization schedules and as required by California law.

ISMP: NAN Alert - Manufacturer’s DexmedeTOMIDine Premixed IV Bags May Be Packaged within An Overwrap Labeled as Acetaminophen

The Institute for Safe Medication Practices (ISMP) has published a National Alert Network (NAN) Alert warning healthcare providers of premixed IV bags packaged with an overwrap labeled for another medication.

The event was reported to ISMP that an infusion bag labeled with an overwrap of “acetaminophen 1,000 mg/100 mL'' was found to be “dexmedeTOMidine hydrochloride injection, 400 mcg/100 mL'' after the infusion was administered. The FDA and the manufacturer, Hikama, have been alerted of the concern, and a formal recall is planned.

Healthcare organizations with these products are recommended to check inventory and sequester products until further instruction. Organizations are to continue the best practice procedure of scanning the barcode directly on an infusion bag and not the overwrap as well as educate staff to be vigilant and pay attention to labels before scanning the barcode of Hikma’s acetaminophen injection.

FDA Approves Treatment for Adults with Alzheimer’s Disease

The U.S. Food and Drug Administration (FDA) has approved an injection treatment for Alzheimer’s disease. Kisunla (donanemab-azbt) is an amyloid beta-directed antibody and is administered as an intravenous infusion every four weeks.

The FDA approved Kisunla (donanemab-azbt) to be initiated in patients with mild cognitive impairment or mild dementia stage of the disease.
The dosage is 700 mg as an intravenous infusion administered over approximately 30 minutes every four weeks for the first three doses, followed by 1400 mg every four weeks.

The safety and efficacy of Kisunla (donanemab-azbt) were evaluated in a double-blind, placebo-controlled, parallel-group study (Study 1, NCT04437511) in patients with Alzheimer’s disease with confirmed presence of amyloid pathology and mild cognitive impairment or mild dementia stage of the disease. Precautions and warnings for Kisunla (donanemab-azbt) include a boxed warning for amyloid-related imaging abnormalities (ARIA). Patients who are ApoE ε4 homozygotes have a higher incidence of ARIA and should be tested for ApoE ε4 status before starting treatment. The most common side effects were ARIA, headache, and the risk of infusion-related reactions.

Free CE from Medscape and MMWR: Pre-Exposure Prophylaxis Among Transgender Women Without HIV Infection: US National Survey 2019-2020

Medscape will be hosting a free online CE activity addressing trends in the use of pre-exposure prophylaxis (PrEP) with current remaining challenges. This activity will help healthcare professionals such as pharmacists to be able to evaluate the use of PrEP among transgender women and variables associated with higher use of PrEP.

Upon completing this activity, participants will assess baseline trends in sexual behavior and transgender healthcare in the current cohort of transgender women, analyze the rates of discussion with healthcare providers regarding PrEP and use of PrEP among transgender women, and evaluate variables associated with increased PrEP use among transgender women and with decreased PrEP use among transgender women.

CDC Recommends Updated 2024-2025 COVID-19 and Flu Vaccines

The CDC released its recommendations for the 2024-2025 Fall/Winter virus season. The CDC recommends those who are ages 6 months and older receive an updated 2024-2025 COVID-19 vaccine regardless of if they have ever previously been vaccinated with a COVID-19 vaccine. This recommendation will take effect as soon as the updated 2024-2025 COVID-19 vaccines become available later this year. The updated 2024-2025 COVID-19 vaccines will be provided by Moderna, Novavax, and Pfizer. Protection from COVID-19 vaccines declines over time and receiving the most updated COVID-19 vaccine will enhance protection against current circulating SARS-CoV-2 variants.

For influenza virus season, the CDC recommends those who are ages 6 months and older, with rare exceptions, receive an updated 2024-2025 flu vaccine. The updated 2024-2025 flu vaccines will all be trivalent and protect against an H1N1, H3N2, and a B/Victoria lineage virus. The CDC encourages providers to start their influenza vaccination planning efforts as soon as possible and to begin vaccinating patients once 2024-2025 influenza vaccines become available based on the following recommendations.

The CDC recommends the months of September and October as the best times for most people to get vaccinated for influenza. Vaccination from July to August is not recommended for most people, except for specific groups such as pregnant people who are in their third trimester and children who need two doses of the flu vaccine. Influenza vaccination should be considered for July to August for children with healthcare visits during that time, adults (especially those 65 years old and older), and pregnant people in the first and second trimesters if there may not be another opportunity for vaccination in September and October.

CDC COCA Call: Update on Highly Pathogenic Avian Influenza A(H5N1) Virus for Clinicians and Healthcare Centers

The CDC will host a COCA Call where presenters will discuss updates on the current outbreak in the United States, current CDC surveillance, and monitoring efforts for the Highly Pathogenic Avian Influenza (HPAI) (H5N1) virus. Highly Pathogenic Avian Influenza (HPAI) (H5N1) virus is that affects birds and in recent cases, has spillover to mammals, such as dairy cattle.

Though the risk is low to the public, those who have job-related or recreational exposure to infected birds or animals are at greater risk of HPAI A(H5N1) virus infection. To date, there have been three human cases of HPAI A(H5N1) virus infection identified in dairy farm workers in the United States.

The objectives of this COCA Call include understanding the epidemiology and clinical features of human cases of HPAI A(H5N1) virus infection, describing the risk of human infection with HPAI A(H5N1) viruses, and identifying the higher risk populations, and what to assess in clinical settings. Additionally, other objectives include describing testing, using antivirals, and infection prevention and control recommendations for patients with HPAI A(H5N1) virus infection.

CDC Updates Recommendations for RSV Vaccinations for Adults

The CDC has updated its recommendations for the use of the respiratory syncytial virus (RSV) vaccines in people ages 60 and older ahead of the upcoming respiratory virus season. Previously, the RSV vaccine was approved for those ages 60 and older. This 2024-2025 respiratory virus season, the CDC now recommends that people ages 75 and older receive the RSV vaccine. This recommendation is for those who have not received an RSV vaccine and did not get the RSV vaccine last year.

For those who are ages 60 to 74, the CDC recommends those who are at risk for severe RSV, which includes those with certain chronic medical conditions, such as lung or heart disease, or who live in nursing homes receive the RSV vaccine.

This updated recommendation is to simplify the RSV vaccine decision-making guidance for clinicians and the public. The RSV vaccine is not an annual vaccine and patients do not need to get a dose every respiratory season. The CDC recommends the best time to get vaccinated for RSV is in the late summer and early fall.
