All Evara Health facilities will remain CLOSED Friday, October 11th.

Due to the updated timeframe for hurricane Milton's landfall, all Evara Health facilities will be closed through Thursday. We will conduct safety and storm damage assessments to decide when normal operations can resume, potentially by Friday morning. Your safety is our priority.

is a time to highlight THE VALUE OF VIRTUAL CARE.

We would love to take this time to spotlight our Mobile Home at Home (MHH) and Remote Patient Monitoring (RPM) programs.

MHH: Patients receive all the benefits of an office visit without having to leave the comfort of their home. We bring all the technology needed for the telehealth visit including both the telehealth and clinical equipment, allowing our team to collect samples for labs, administer EKGs and more!

MHH for Women’s Health: Postpartum and Newborn Visits. Normally Women 21-30 for an average of about 90 days on the program. Our Medical Home at Home Women's Team can even coordinate a combo visit with mom and baby together.

RPM: For at risk patient populations. Fantastic for managing chronic conditions like high blood pressure, diabetes, etc. We provide patients with all the equipment needed to take their daily vitals, this data then goes directly to the RPM Portal where it is monitored by our Nurses - any at risk vitals can be identified in the case that escalation or intervention is needed.

RPM for Women’s Health: Pregnant and Postpartum Moms. Especially at risk for gestational diabetes and gestational hypertension.

We witness firsthand THE VALUE OF VIRTUAL CARE. When patients see daily vitals, it encourages them to take control of their health, making better decisions and leading to healthier outcomes!
