Once you have completed registration, your doctor will be notified and depending on his/her discretion, they will select your next appropriate follow-up interval. You will be notified of this and closer to the time, you will receive a notification. You can then log in and complete a simple follow-up guided assessment tool to help your doctor decide how well controlled your chronic illness is.If you make use of any remote monitoring devices e.g. glucose monitors, you can share any information which will give your doctor a more detailed impression of how well controlled you are.

No problem. In life, we have choices, and should you decide to see a different doctor, your profile can be linked to another practitioner by editing your profile within ChronicDoc. This is simply done using your practitioners unique invite code. You will then receive notifications from that practitioner. Simple - just the way life should be
