Oct 20 is World Osteoporosis Day which is a fitting time to share some recent cool data from The Bone Clinic in Brisbane. The LIFTMOR trial showed 2x week ONERO was enough to increase bone mass, so we consider 2x week ONERO for 48 weeks a year (allowing 4 weeks for holidays, illness etc) to be 100% adherence (attendance). People often ask me if once a week is enough to cause bone benefit or if 3 times is better than 2. You need a lot of data to answer those questions and after 9 years we finally have it. Look at this plot showing % change in lumbar spine BMD according to adherence. Some people are attending >100% which means 3 times a week. Interestingly you can see that just once a week still had an effect but 2 and 3 times improved bone more. Cool huh? Happy World OP Day from TBC!
Important economic analysis published yesterday of the growing burden of osteoporotic fracture in Australia. The grim reality of this 'silent disease' is that it's not only a blight on quality of life and longevity of older adults, it's hitting ALL Australian tax payers in the hip pocket. Big time. 1 hip fracture alone costs >$47,000! Medicare reimbursement of ONERO would help!

It is well known that that loss of muscle, especially leg muscle, increases risk for osteoporotic fractures. This study, including data from over 20,000 people, shows osteoporosis medication does not reduce that risk. Take home message, bone drugs act on bone. But falls are the main reason people fracture a hip. Only exercise prevents falls. Keep lifting folks.

Really enjoyed reconnecting with like minds on my other love this week - bone stress injuries - at the World Congress on Military Medicine in Brisbane. Even before deployment, defence recruits around the world often fight their first battle with overuse injuries in recruit training. Our PREFIT project (pretraining before Army) reduced injuries during recruit training from 61% to 19% so we have the knowledge. The challenge is to implement.

My second quality assurance stop in Perth was Chronic Care Australia in Cottesloe where I forgot to get a photo in the clinic. Luckily Director Katie Stewart spoiled me with dinner at the local Lamont's restaurant (superb) which was an even better photo op. But the over-large bottle is not doing anything to dispell my newly realised sense of shortness.
My first stop in Perth on my Onero quality assurance visit was Mead Physio in Kalamunda where the wonderful Ned Brennan ran a great class with 5 highly motivated and grateful ladies. Didn't realise what a midget I am until I saw this photo!

Just when you thought I was going to say something different...noop. Just more evidence you're never too old to reap the rewards of resistance training. And on to California. We had a great in-service on a very hot afternoon, kindly hosted by new Licensee Patrick Smith at Evolve Fitness in Los Gatos. Delighted to also be joined by our very first Californian Licensee, Wes Anderson, from Helix PT in Danville, as well as Jill Rytand from Los Altos and our 2 new Licensees from San Diego, Deb Serban of Serban Wellness and Fitness and Wendy Green of Phyzique Multisport physical therapy. My only regret is Patrick's shoulders were too broad for his Onero T shirt!
Next stop, Canada. Took a sidetrip to the University of Waterloo to visit Prof Lora Giangregorio, who does research in the same field as me at the opposite end of the world. Lora was an incredible host and tour guide, including a lovely walk along the river. Talking shop with like minds is one of the joys of academia. We don't always agree but if we did, it wouldn't be science!

The North American journey continued to Manhasset (Long Island) where I visited our first New York Onero Licensee, Marni Larkin and her crew at Gyrotonic Manhasset Physical Therapy. Marni is dedicated to evidence-based practice and so licensed all 7 of her staff. I QAed some classes, ran an in-service that included other Licensees from New York, and gave a talk the next day for interested patients and clinicians. Marni was an incredible host, including taking me for cocktails in Manhatten, and I can't wait to go back ...I need to check on them again ...you understand. ;)
The next stop was Long Island where I stayed with the fabulous (and quite frankly, legendary in the world of mechanical loading of bone) Professor Clint Rubin and his wonderful wife Jenni. Clint and I spent a couple of days working on our data from the VIBMOR trial. For those who are anxiously awaiting the results of that trial examining the effects of vibration on bone and other risk factors for fracture, it will be a little while yet as we pore through many, many outcomes. Trust me, when we have results I will disseminate widely, including here.
June was North America month and it began in Boston (actually end of May) at the annual scientific meeting of the American College of Sports Medicine where I presented The Bone Clinic data. Lots of interest. Boston is a fabulous town.
It can be frustrating when hard fought gains in fitness or strength evaporate when you stop training, but a cool new study shows it might not be the case if you lift heavy enough. In a rare examination of long-term effects of 1 year of supervised heavy resistance training, retirement age adults sustained long-lasting (3 years) benefit in leg strength. Notably, strength decreased in both the moderate-intensity training and non-exercising control groups over the same time period.

And just when you thought there couldn't possibly be more advantages to Onero... "Resistance training (RT) participation is associated with reduced all-cause and cancer-related mortality and reduced incidence of cardiovascular disease, hypertension, and symptoms of both anxiety and depression" And the evidence shows low-volume, high-load RT is very effective. Sound familiar? (published in 2022 but it doesn't hurt to remind everyone why we're lifting so heavy!)

Our approach to helping people build strong bones and preventing osteoporotic fractures involves more than just delivering evidence-based exercise and dietary advice. Raising awareness of this effective therapy in the clinical community is vital and what better way than to show doctors in training? Delighted to be a placement site for 2nd year UQ Medical students last semester, and going again in Sem 2.

While The Bone Clinic will have a primary focus on the prevention and management of osteoporosis, including client education, nutritional counselling and evidence-based exercise therapy. The prevention and management of other bone conditions such as stress fractures will also be addressed.
