Reliability and Usefulness of the Single Leg Heel Raise Balance Test in Patients with Chronic Ankle Instability⠀⠀⠀⠀⠀⠀⠀⠀⠀

Researchers sought to analyze the differences in static (including conventional and modified [single-leg heel-raise balance]) and dynamic postural stability and muscle endurance between patients with chronic ankle instability (CAI) and healthy controls and to determine the reliability and usefulness of the single-leg heel-raise balance test in patients with CAI. Twenty-six patients with CAI and 26 healthy controls were enrolled. Postural stability was assessed using a postural stabilometry system. Muscle endurance was measured in dorsiflexion and plantarflexion using an isokinetic device. Modified static postural stability and dynamic postural stability were significantly poorer in the affected ankles of patients with CAI than in the controls. Plantarflexion endurance was significantly lower in the affected ankles of the patients with CAI than in the controls. Modified static postural stability significantly correlated with plantarflexion endurance in both groups. Researchers said given the reliability of the modified static postural stability test, clinicians and therapists should consider using it to assess improvements in postural stability and muscle endurance in patients with CAI before and after rehabilitation.⠀⠀⠀⠀⠀⠀⠀⠀⠀

Literature Review:
Unicameral bone cysts (UBC) are common benign non-neoplastic lucent bone lesions filled with fluid that enlarge over time and thin the adjacent bone cortices (1, radiopaedia.org) These lesions are almost exclusively found in children and adolescents between the ages of 3 to 14 years of age, and are over twice as frequent in males than females (Mascard). UBCs are typically found in the metaphysis of long bones with an open physis especially the proximal humerus and femur; however, they have been diagnosed in almost every bone including tarsal bones and the calcaneus. (2) Though rare, UBCs can be seen in unusual locations such as the talus (radio*).

UBCs are most often asymptomatic and are incidental findings and may resolve on their own as a patient approaches skeletal maturity. The most frequent clinical presentation of a symptomatic UBC is pain, stiffness, swelling, with a concomitant pathologic fracture (Mascard). The pathogenesis of this cyst is largely unknown, though it is postulated to arise as a defect during bone growth that creates a cavity filled with fluid. The cyst is located adjacent to the physis during its active phase, and migrates away when it is latent (**). Because the cortical bone is weakened by the cyst, a traumatic event can result in a pathologic fracture.

Mini-open Arthroplasty for Anterior Ankle Impingement

Osseous or soft tissue impingement of the anterior ankle can result in reduced dorsiflexion, pain and difficulty in walking that can significantly reduce the quality of life. Sometimes, conservative treatments fail & surgical intervention may be indicated.
⠀⠀⠀⠀⠀⠀⠀⠀⠀
A 34-year-old male, who sustained a severe ankle injury five years earlier whilst waterskiing, complained of ongoing pain & disability. His initial injury had resulted in multiple fracture fragments within the anterior & posterior aspects of his right ankle. Upon performing clinical examination, reduced dorsiflexion with crepitus was noted and plain film radiographs exhibited multiple fragments within the anterior & posterior aspects of his ankle joint. Pain & limitation to motion were primarily affecting the anterior joint. The posterior aspect was only mildly symptomatic & not considered a primary consideration. Treatment involved a mini-open arthroplasty with removal of the osteophytes & margination of the chondral defects. Adequate dorsiflexion without crepitus was noted on the operating table & postoperative radiographs showed adequate removal of the osteophytes. He was kept non-weight bearing in a fibreglass back slab for 10 days before resuming partial weight bearing in a postoperative shoe & crutches.

Mini-open arthroplasty may offer a viable alternative to large incision joint procedures or arthroscopy & may be a procrastinatory procedure for patients wishing to delay ankle joint arthrodesis or replacement.
