Scientists Just Uncovered A Whole New Layer of Brain Anatomy 05/01/2023

Interesting research published in Science about new learnings of brain immunity, lymphatics and vasculature.

Abstract: The central nervous system is lined by meninges, classically known as dura, arachnoid, and pia mater. We show the existence of a fourth meningeal layer that compartmentalizes the subarachnoid space in the mouse and human brain, designated the subarachnoid lymphatic-like membrane (SLYM). SLYM is morpho- and immunophenotypically similar to the mesothelial membrane lining of peripheral organs and body cavities, and it encases blood vessels and harbors immune cells. Functionally, the close apposition of SLYM with the endothelial lining of the meningeal venous sinus permits direct exchange of small solutes between cerebrospinal fluid and venous blood, thus representing the mouse equivalent of the arachnoid granulations. The functional characterization of SLYM provides fundamental insights into brain immune barriers and fluid transport.

Quantified hemodynamic parameters of the venous system in multiple sclerosis: A systematic review 26/01/2022

New Australian review published in Multiple Sclerosis and Related Disorders (MSARD). It summarises the quantified values of cerebral venous blood flow and pressure which has been measured in the literature. The findings were that there was a lack of information regarding blood flow in the intracranial regions of the cerebral venous system, as well as pressure more generally. This shows the need for further research.

CCSVI Australia is funding further research with this same group over 2022-23. More details to come.

It is with sadness that we learnt today of the passing of Peter Sullivan.

I first spoke with Peter 11 years ago about the unfolding CCSVI research and real world experiences of people with MS. His warmth, honesty, generosity, integrity and quiet strength is what I will remember as he championed the work of people with MS and open-minded MS researchers both here in Australia and across the world who were investigating the vascular connection to Multiple Sclerosis. He spent the last decade or more documenting and providing commentary as well as directly challenging the status quo and existing dogma held by our MS institutions, neurologists and government.

If I could sum up what I personally learnt from Peter it would be that people with MS deserve to have their experiences and perspectives heard, understood and respected and their rights upheld - anything less is simply unacceptable. He lived this value right up to his last days.

The world has lost a true gentleman and our sympathies go out to Peter's family and friends.

Does the venous pressure theory of Multiple Sclerosis pathophysiology deserve a second chance? Australian researchers G Bateman et. al. say 'yes.' Check out their response to Beggs and Zamboni's recent comments.

Summary:

The theory that multiple sclerosis is related to venous pressure has been discredited due to previous operator dependent diagnostic criteria and premature attempts at treatment. (1) An elevation in venous pressure may only be a component of the compliance changes found in MS. (2) The neck veins may only supply a component of the venous pressure elevation found intracranially. Although a more targeted approach towards neck angioplasty (both towards disease subtype and those with more favourable stenoses) may be beneficial, we would advocate caution. We encourage others to give the venous pressure theory a second chance and to replicate our work.

Clive Beggs and Paolo Zamboni provide comment on recent paper by Australian researchers. Summary:

Bateman et al. (2021) shows that multiple sclerosis (MS) is strongly associated with raised pressure in the superior sagittal sinus (SSS) and increased jugular bulb height above the sigmoid sinus. These findings are consistent with an increased aqueductal CSF pulse, as previously described in MS. They reinforce the hypothesis that intracranial compliance is reduced in MS and that internal jugular vein abnormalities contribute to SSS hypertension. However, the contribution of this to the pathophysiology of MS has not been established. Further investigation is therefore needed to determine what role, if any, these changes play in the complex puzzle of MS.

Possible Markers of Venous Sinus Pressure Elevation in Multiple Sclerosis: Correlations with Gender and Disease Progression 17/08/2021

New paper from Australian researchers investigating differences in venous structures, pressures, grey matter loss and disability severity in people with MS.

Key findings:

Males with MS and those with progressive forms of MS showed greater grey matter volume loss and larger EDSS scores compared to females with MS and those with RRMS.

In patients with MS, worse outcomes in both males and progressive forms of MS were associated with larger sagittal sinus cross-sectional areas suggesting altered sinus pressure.

Females with MS have narrower transverse sinuses than males but males have higher jugular bulbs.
