I have organized a workshop this Friday (19th) evening and am giving away a lot of ideas that I have picked up on the topic over the years and 500+ presentations at various forums.

The idea that slides are an essential part of all presentations is a myth. Just look at TED talks, where the majority of speakers don't use slides at all. Then how do they spread their ideas worth sharing? I will discuss some great concepts in this regard.

A two minute primer on mendelian randomisation 19/12/2023

Have you heard about Mendelian Randomization (MR) and how it can be used to study causality between risk factors and disease? See this interesting 2-minute YouTube video to know what MR is.
In summary, everything we do including how much we eat, exercise, sleep and smoke/drink is determined by our genetics (that's the premise of MR). We can identify genes that are related to less and more smoking, and as it turns out, those with genes for more smoking die earlier, thereby confirming that Smoking kills.
We might have MR analysis that shows a predisposition to cancer (already BRCA1,2), diabetes, retinopathy etc, and how it could influence incidence and disease outcomes, thereby paving the way for personalised medicine.
It's not that far away with large-scale Artificial intelligence models capable of performing Genetic summary statistics from available genome-wide association study databases and MR showing disease associations.

Diabetic retinopathy can cause blindness on diabetics if not detected on time.

These are images of regressing neovascularization at the disc (NVD) in both eyes of the same patient with proliferative diabetic retinopathy (PDR) I.e abnormal blood vessels and bleeding in the retina of the eye. This is a 50-year old gentleman.

RIGHT EYE - the entire NVD has almost disappeared post PRP laser after 3 months. See the annotated image (yellow) below the main image outlining the NVD for better understanding.

LEFT eye - The large NVD has consolidated and shrunk and heaped up closer to the disc after 3 months of PRP. See the annotated image (yellow) outlining the NVD.

The real question is, do all NVD and NVE disappear completely after PRP laser? The answer is NO, all neovascularization does not disappear. Some do, without leaving a trace, some shrink but persist, and some just don't change in nature at all.

Periodic photographic documentation and comparison will help you understand the regression patters and avoid unnecessary repeat lasers, which should be reserved only for those with growing NVD/NVE despite good PRP.

Mucormycosis facts and myths:

There is a lot of posts about BLACK Fungus going on WhatsApp and eye specialists are getting bombarded with questions. So writing this to improve understanding of the

Mucormycosis predominantly happens to diabetics (largely older) who are poorly controlled and have taken steroids due to COVID which shoots up blood sugar to very high levels.

It usually happens after recovery from COVID, when blood sugar is exceedingly high post steroids use.

It first affects the sinuses and will cause blocked nose and nasal discharge, often blood stained or dark red (almost black). You might also see black spots in the nostrils or on the hard palate inside the mouth (the hard upper aspect)

It is treated by ENT surgeon as it's a sinus infections.

If it spreads beyond the sinus, it involves tissue around the eye, the first symptom is usually dropping of upper eye lid and seeing double vision when both eyes are open.

Next, eye movements slow down in the affected side.

Only then you get blurred vision, by which time the infection is advanced and will likely need aggressive treatment.

If caught at the right time by ENT SURGEON, a procedure called FESS (Google full form if interested) is done, infection removed and anti fungal treatment given which can salvage life.

Mucormycosis is a very rare condition and is happening more these days because of heavy and often unnecessary use of high dose steroids in diabetics.

Having COVID does not increase risk of Mucormycosis. But being an uncontrolled diabetic on steroids definitely increases risk many fold, with or without COVID.

Take home message:
1. Use steroids judiciously in diabetic,

2. Do not self medicate,

3. If using steroid, then monitor blood glucose closely.

4. If you are diabetic and have taken steroids, then be watchful for blocked nose and discharge and droopy eye lid a few days after COVID recovery.
