Recenti ricerche hanno fatto luce su due tipi di carcinoma a cellule squamose legati al seno: uno che inizia nel tessuto mammario stesso (PSCCB) ed uno che origina dalla capsula intorno agli impianti mammari (BIA-SCC).

🔸**Punti Chiave:**
- Il PSCCB è più comune, con circa 2 casi ogni 100.000 persone.
- Il BIA-SCC è molto più raro, con circa 0,61 casi ogni 100.000 persone.
- Il rischio di avere il PSCCB è di 1 su 49.509 donne, mentre per BIA-SCC è di 1 su 164.884 donne con impianti.

🔸**Confronto con altri rischi:**
- Il BIA-SCC è molto meno comune rispetto all’altro cancro legato agli impianti, il BIA-ALCL, che si verifica in 1 su 914 donne con impianti testurizzati (USA 🇺🇸)

🇬🇧 **Important Update on Breast Cancer and Breast Implants in my new paper: „Breast Implants and the Risk of Squamous Cell Carcinoma of the Breast: A Systematic Literature Review and Epidemiologic Study”** 📖

Recent research has shed light on two types of squamous cell carcinoma linked to the breast: one that starts in the breast tissue itself (PSCCB) and another associated with breast implants (BIA-SCC).

🔸**Key Findings:**
- PSCCB is more common, with about 2 cases per 100,000 people.
- BIA-SCC is much rarer, with about 0.61 cases per 100,000 people.
- The lifetime risk for PSCCB is 1 in 49,509 women, while for BIA-SCC, it's 1 in 164,884 women with implants.

🔸**Comparison with Other Risks:**
- BIA-SCC is much less common than another implant-related cancer, BIA-ALCL, which occurs in 1 in 914 women with textured implants (USA 🇺🇸).

🔸**What This Means for You:**
- Both PSCCB and BIA-SCC are rare, but it's important to stay informed about potential risks.
- If you have breast implants, especially textured ones, keep regular check-ups with your doctor and discuss if not to change it.

Ho avuto l'onore di cedere la presidenza della Società alla mia cara collega, Prof.ssa Efterpi Demiri 🏅 C'è stato anche il tempo per festeggiare e ballare con il mio amato figlio. I viaggi di famiglia ai congressi possono essere così piacevoli 🥰

🇬🇧 It's been a month since the memorable EURAPS (European Association of Plastic Surgeons) congress in Athens. It was a congress full of wonderful lectures, interesting ideas, new friends, and unforgettable parties. I had the honor of handing over the presidency of the Society to my dear colleague, Prof. Efterpi Demiri 🏅 There was also time for celebrating and dancing with my beloved son. Family trips to congresses can be so enjoyable 🥰

🇮🇹 Sono stato felice di tornare in Polonia per tenere lezioni e operare durante il Masterclass „Lights & Shadows of Plastic Surgery”, organizzato dal Dott. Jerzy Kolasiński. Insieme al Prof. Michail Sorotos abbiamo mostrato ai partecipanti come eseguire un lembo di gran dorsale integrato con tessuto adiposo (un lembo dalla parete toracica posteriore per ricostruire una mammella. Magia! 🧙🏻‍♂️)

Dopo la Masterclass, c'è stata la festa per il 40º anniversario della clinica del Dott. Kolasiński. Era piena di attrazioni e ho anche avuto l'opportunità di volare in mongolfiera 🎈

🇬🇧 I was happy to be back in Poland to give lectures and operate during the Masterclass „Lights & Shadows of Plastic Surgery”, organized by Dr. Jerzy Kolasiński. Together with Prof. Michail Sorotos we showed the participants how to perform a Fat Augmented Latissimus Dorsi Flap (a flap from the posterior thoracic wall to reconstruct a breast mound. Magic! 🧙🏻‍♂️)

After the Masterclass, there was the party for the 40th anniversary of Dr. Kolasiński's clinic. It was full of attractions! I even had the opportunity to fly a balloon 🎈

Lì sono stato lieto di presentare al convegno Chirurgia Ricostruttiva ed Estetica della Mammella la tecnica che ho inventato, Fat Augmented Latissimus Dorsi Flap. 🇬🇧One of the examples of my stressful, busy life: last week I landed from Boston in the morning, and went to check my unit in the hospital, immediately afterwards I repacked at home and the same day in the afternoon I flew to Fossano, near Turin. There I was pleased to give my lecture about Fat Augmented Latissimus Dorsi Flap, which I invented, at the meeting Chirurgia Ricostruttiva ed Estetica della Mammella. 🇮🇹 È stato un mese molto impegnativo, pieno di viaggi e lavoro. Sono stato prima al 102º Congresso Annuale dell'American Association of Plastic Surgeons a Boston, dove ho avuto il podio per la settima volta consecutiva. Un enorme privilegio essere selezionato per presentare il mio studio „Breast Implants and the Risk of Squamous Cell Carcinoma of the Breast.” Questo era uno studio epidemiologico di revisione sistematica della letteratura che ha dimostrato che non esiste una relazione causale tra le protesi mammarie e questa grave condizione.

ℹ️ BIA-SCC è una malignità che origina nella capsula intorno all'impianto, molto probabilmente dopo un'infiammazione cronica dovuta all'attrito, ma può anche verificarsi senza impianto (SCC della mammella) a causa di un'infiammazione cronica della ghiandola mammaria stessa.

🇬🇧 It was a very busy month, full of travels and work. I was first to the 102nd Annual meeting of American Association of Plastic Surgeons in Boston, where I’ve got the stage for the seventh consecutive time. An enormous privilege to be selected to present my paper „Breast Implants and the Risk of Squamous Cell Carcinoma of the Breast” This was a systematic review and epidemiologic study that demonstrated that there is no causal relationship between breast implants and this serious condition.

ℹ️ BIA-SCC is malignancy occurring in the capsule around the implant, most probably after chronic inflammation due to friction , but it may also occur without implant due to chronic inflammation of the breast gland itself.

🇮🇹 Ieri il NCCN ha pubblicato un aggiornamento sulle linee guida per il BIA-ALCL, ribadendo e spiegando ulteriormente quanto già dichiarato dall’FDA. Nei pazienti asintomatici con protesi testurizzate, l'espianto non è generalmente raccomandato. Tuttavia, dopo una stratificazione del rischio, potrebbe essere ragionevole considerare l'espianto in casi selezionati per ridurre il rischio. In pratica ricopiano le conclusioni dei miei lavori scientifici. Sono orgoglioso che il NCCN abbia citato addirittura 3 delle mie ricerche scientifiche 🤩

Ora non so se citare in giudizio il NCCN per plagio o ringraziarli per aver utilizzato le conclusioni dei miei articoli! 😂😂😂

🇬🇧 Yesterday the NCCN* published an update on the guidelines for BIA-ALCL, reiterating and further explaining what was already stated by the FDA. In asymptomatic patients with textured implants, explantation is not generally recommended. However, after risk stratification, it may be reasonable to consider explantation in selected cases to reduce the risk. They practically echoes the conclusions of my work. I’m proud that NCCN* refered to as many as 3 of my scientific researches 🤩

Now I don’t know if to sue the NCCN for plagiarism or thanks them for using the conclusions from my papers! 😂😂😂

I’m happy to share with you that two talented young doctors, Dr Nandini Singh Tanwar from India and Dr Hud Shaker MD PhD from the United Kingdom, who have just completed a two-week visit at Sant’Andrea University Hospital in Rome, came back home. During their time with me, we had the opportunity to delve into some fascinating cases, including primary DIEP flap breast reconstruction, but also secondary cases after recurrent chest wall sarcoma, or contralateral breast symmetrization with fat transfer. I hope that our colleagues found their training experience enriching and rewarding.

Further, I’m pleased to announce that we are now offering shorter-term opportunities to attend our Fellowship in Clinical and Experimental Reconstructive Microsurgery. Recognizing the demands of modern life, we have opened up options for periods of 2 weeks, 1 month, and 2 months. This flexibility aims to accommodate young doctors who may have family or work responsibilities, making it easier for them to join us 🤩

🇮🇹 Sono assolutamente entusiasta di essere stato parte del corpo docente dell'ASPS Spring Meeting di ieri, dove ho avuto il privilegio di presentare una relazione dal titolo „Linee guida europee per il trattamento del BIA-ALCL” discutendo le indicazioni per la capsulectomia nella chirurgia dell’espianto.

Alla luce delle crescenti preoccupazioni riguardanti il BIA-ALCL e la BII, questo argomento non potrebbe essere più tempestivo o cruciale. Come chirurgo plastico e ricercatore dedicato, credo fermamente che il nostro impegno nel far progredire la conoscenza vada oltre i confini dei nostri doveri. Mentre alcuni potrebbero vedere i fine settimana come un momento di relax, per me è un'altra opportunità per contribuire alla comprensione collettiva del nostro settore.

🇬🇧 I am absolutely thrilled to have been a part of the esteemed faculty at yesterday's ASPS Spring Meeting, where I had the privilege of delivering my lecture titled „European Consensus Guidelines for Treatment of BIA-ALCL” questioning Indications for Capsulectomy in Explant Surgery.

In light of the growing concerns surrounding BIA-ALCL and BII, this topic couldn't be more timely or crucial. As a plastic surgeon and dedicated researcher, I firmly believe that our commitment to advancing knowledge extends far beyond the confines of our offices. While some may see weekends as a time for relaxation, for me, it's another opportunity to contribute to the collective understanding of our field.

I am grateful for the opportunity to share my insights and expertise at such a prestigious event, and I look forward to continuing the dialogue and collaboration with ASPS - American Society of Plastic Surgeons (ASPS)
