Documentary Requirements:
1. Filled-up application form
2. 2x2 picture with white background
3.Transcript of Records (Original or Certified true copy)
4. PSA Birth Certificate (upon Admission)
5. Honorable Dismissal/Certificate of Transfer
Credentials (upon Admission)
6. Original marriage contract (for married female
applicant only)
7. Non-refundable testing fee of ₱250.00
Procedure
1. Download and print the application form. Provide the necessary information. (Note: Downloadable Application form is available on this link: https://drive.google.com/file/d/1P0lDztw_eLU1cj-6B8X8rkCSb-XzqTas/view or you may scan the QR code below to download )
2. Proceed to the College and submit all the requirements for evaluation.
3. Proceed to Testing and Admission Office and submit the evaluated application form to secure admission test permit.
4. Pay 250 pesos testing fee at the Cashiering Office and present the receipt to TAO.
5. Take the GSAT on the schedule indicated in the test permit.
6. Present the Test Permit and one valid I.D to claim the Notice of Admission.
