1. Duly accomplish registration form
2. Medical Certificate Copy from University Medical
3. 2x2 picture 2 copies ( white background with nametag)
format: Surname, Full name Middle Name
4. Certificate of Registration

Note: If the original Duly Accomplish Registration Form and Medical Certificate are unavailable, you may submit a photocopy of your documents.
