In partnership with the Land Transportation Office (LTO), the Sangguniang Bayan of San Jose is excited to offer a Free Theoretical Driving Course for aspiring drivers in our community! This initiative aims to promote road safety and equip participants with essential driving knowledge.

- Duly accomplished Application for Permits and Licenses (APL) Form (will be filled out during the
course)
- Original and one (1) photocopy of any issued ID with photo and signature of the applicant - Original and one (1) photocopy of any of the following:
1. PSA / NSO/ Local Civil Registry Birth Certificate
2. Valid Philippine ID
3. Passport
4. PSA Certificate of Marriage (For married female applicant)
- For applicants below 18 years of age:
1. Parentβs consent or a person having custody/care or supervision over the minor applicant
authorizing the applicant to secure a student permit
2. Original and photocopy of any valid government-issued ID of the parent/affiant with photo and
signature

- LTO Application for Student Permit Processing Fee: PHP 250.00
- Medical Clearance (from LTO Accredited Medical Clinic): PHP 600.00

- Only 200 online and walk-in pre-registered applicants will be entertained.
- Applicants must meet the minimum age requirement (16 years of age and above).
- Applicants must be physically and mentally fit.
- Applicants must be able to read and write in Filipino or English.
- Applicants must not have unsettled traffic violation.
- Applicants must have an active and valid email address.
- Applicants must finish the 15-hour seminar to obtain the Theoretical Driving Course Certificate.
- Please wear proper attire during the seminar. Shorts, sleeveless tops, and slippers or sandals are
not allowed.
