As advancements in systemic therapy continue to improve survival of patients with metastatic cancer, the frequency of patients requiring radiation therapy for treatment of brain metastases has increased. Radiotherapeutic management of brain metastases can range from radiosurgery to whole brain radiation with or without hippocampal sparing.

What are some of the considerations in selecting patients for stereotactic radiotherapy, and what dose fractionations should we use? How should we approach and contour leptomeningeal disease, skull metastases and the hippocampus? How do we differentiate post-treatment radiation changes from disease progression on surveillance imaging?

Stereotactic body radiation therapy (SBRT) is commonly used for peripheral lung tumours with good outcomes and most radiation oncologists are comfortable in treating such lesions. However, when faced with a central/ultra-central lung tumour, SBRT becomes more challenging. Some trials have reported severe toxicity or even fatalities with central lung SBRT. Others have proposed risk adapted fractionation with resultant lower BEDs delivered to the target lesion. This has caused radiation oncologists to shy away from treating any tumours within the "no fly zone".

Is this fear unfounded? Should we just hand over the baton to our surgical colleagues in these cases? Or is there a safe and proven approach to deliver central lung SBRT, thus allowing the patient to harness the advantages of extreme & precise hypofractionation?

Join us as we listen to our expert panel dissect the issues in managing central lung tumours. Professor Suresh Senan, who was a key investigator in the SABR-COMET trial, will take us through his considerations when faced with a central lung tumour and how to treat such tumors safely while Dr Harish Mithiran, our cardiothoracic surgical colleague will enlighten us on how they manage such tumours and when SBRT would be preferred. Finally we will have our principal radiation therapist & principal dosimetrist, Ms See Peishi and Mr Cai Shao Bin respectively who will share their allied health perspectives on delivery of SBRT lung treatments.

Prostate SBRT is an elegant, effective and efficient way to treat prostate cancer that now has a place in internationally recognized guidelines. This segment of SPOT aims to also make it easier for centres interested in adopting prostate SBRT for their patients.

The precision required for Prostate SBRT mandates the use of MRI to aid target and OAR delineation, bringing in complexity beyond fractionated treatment contouring. Hence the focus will first be on radiology, specifically MRI, to equip radiation oncologists with the necessary background knowledge about Prostate MRI sequences, anatomy and lesion recognition.

Beyond sharing of clinical expertise and experience in delineation of target volumes / OARs for prostate SBRT, hot button issues and controversies will also be discussed. Who are the suitable candidates for prostate SBRT - can it become the de facto standard for external beam radiation? Does ADT integrate with SBRT in the same way as conventionally and moderately hypofractionated RT? Is nodal irradiation using SBRT ready for prime time?

Dosimetry also comes to the fore in this edition of SPOT and features planning tips and tricks on how best to optimise plans for Prostate SBRT, with or without the use of peri-rectal spacers which have become more commonplace in recent years.
